摘　　要：目的 在二胎政策放开的情况下,探讨随着妊娠次数增加,O型孕妇产前IgG抗A（B）效价与新生儿溶血病发生率相对初产妇有所改变,建议提早及增加IgG抗A（B）效价检测频率的必要性。方法 对2016年1-7月符合入选标准的200例新生儿ABO溶血病先区分成首次妊娠组和非首次妊娠组,对临床资料和实验结果进行回顾性分析。结果 首次妊娠组与非首次妊娠组的两组孕妇IgG抗A（B）效价具有显著性差异（P〈0.05）。两组患儿改良直接抗人球蛋白试验、抗体释放试验、游离抗体试验结果相比较具有显著性差异（P〈0.05）,ABO新生儿溶血病发病率及病情与妊娠次数的增加呈现一定相关性。结论 对于非首次妊娠组的患儿更容易发生新生儿ABO溶血病,而非首次妊娠组的母亲特别是二胎孕妇更需要在孕中期提前进行产前动态检测血型IgG抗体效价。这对于预防新生儿ABO溶血病的发生具有重要的意义。Objective To explore prenatal IgG anti-A（B） potency in O-type pregnant woman with the increase in the number of pregnancies and the incidence of neonatal hemolytic disease have some difference compared with primipara in the case of second child policy liberalization, and the necessity of frequency of detecting IgG anti-A（B） potency are to be advised. Methods A total of 200 cases of neonatal ABO hemolytic disease meeting the inclusion criteria from Jan- uary to July 2016 were divided into the first pregnancy group and the non-first pregnancy group, then the clinical data and experimental results were retrospectively analyzed. Results The anti-A（B） potency of the first pregnancy group and the non-first pregnancy group was significantly different（P〈0.05）. There were significant differences in modified direct anti-human globulin test, antibody release test and free antibody test results between the two groups（P〈0.05）. ABO neonatal hemolytic disease morbidity and conditionthe had a certain correlation with the increase in the number of pregnancies. Conclusion For non-first pregnancy group of children who are more prone to have newborn ABO hemolytic disease, and mothers in the first pregnancy group, especially the pregnant women with a second child need to undergo prenatal dynamic detection of IgG antibody potency of blood type in the second trimester in advance.
