枸橼酸钠封管液预防血液透析深静脉置管感染的临床观察

摘　　要：目的:比较4%枸橼酸钠与头孢唑林钠肝素钠混合液两种封管液对预防血液透析患者深静脉置管相关感染的效果.方法:选取2012-05/2014-06在江苏大学附属医院肾脏科接受血液透析治疗,右侧颈内静脉长期置管患者91例作为研究对象,根据患者导管封管液的不同,将其分为观察组(n=49,4%枸橼酸钠组)和对照组(n=42,头孢唑林钠肝素钠组),分析两种不同的封管液预防相关感染的的效果及导管通畅性.结果:观察组深静脉置管患者导管相关感染率为2.13%,对照组深静脉置管患者导管相关感染率为2.48%,两种封管液封管的静脉置管发生导管相关感染率比较,差异无统计学意义(P>0.05);两组导管通畅性比较,差异无统计学意义(P>0.05).结论:采用枸橼酸钠封管法和头孢唑林钠肝素钠两种封管液对预防血液透析患者深静脉置管相关感染的效果基本一致,枸橼酸钠封管法更加安全、经济,值得推广.AIM： To compare the effect of tube-sealing 4% solution sodium citrate and mixed liquor with cefazolin sodium and heparin sodium in preventing deep vein catheter-related infections of patients with hemodialysis. METHODS ： A total of 91 cases of patients with hemodialysis and long-term deep vein catheterization on right internal jugular admitted into department of nephrology, Affiliated Hospital of Jiangsu University from May 2012 to June 2014 were selectd as the objects of study and divided into observa- tion group （n = 49, 4% solution sodium citrate） and control group （n= 42, mixed liquor with cefazolin sodium and heparin sodium） according to different tube-sealing solution. The effect of two different kinds of tube-sealing solution in preventing deep vein catheterization-related infections and smoothness of catheter was analyzed. RESULTS： The catheter-related infection rate in the observation group was 2. 13%, while that of control group was 2.48%, with no statistically significant difference （P〉 0.05 ）; There was no statistically significant difference in smoothness of catheter between the two groups （P〉 0.05）. CONCLUSION ： It appears to be that tube-sealing solution sodium citrate and mixed liquor with cefazolin sodium and heparin sodium share the same effect in preventing deep vein catheter-related infections of patients with hemodialysis. Tube-sealing solution sodium citrate is much more safe and economic, which is worth promoting.
