摘 要:目的了解2010-2014年上海市382株志贺菌对抗菌药物的敏感性及其毒力基因的携带情况。方法采用纸片扩散法(kirby-bauer,K-B)检测139株福氏志贺菌和243株宋内志贺菌对13种抗菌药物的敏感性;同时对志贺菌的五种毒力基因进行聚合酶链式反应(polymerase chain reaction,PCR)扩增检测。结果菌株对萘啶酸、链霉素、氨苄西林、四环素和复方新诺明的耐药率分别达到了97.4%、96.1%、84.8%、77.7%和77.2%,多重耐药情况严重。福氏志贺菌对氨苄西林、头孢噻肟、头孢吡肟、氯霉素、阿莫西林/克拉维酸、四环素、环丙沙星、诺氟沙星及左氧氟沙星的耐药率均高于宋内志贺菌,差异均具有统计学意义(均有P<0.05)。382株志贺菌全部携带ipa H基因,其中63.3%的福氏志贺菌同时携带ipa H、ial、sen、set1A和set1B这5种毒力基因,50.6%的宋内志贺菌同时携带ipa H、ial和sen这3种毒力基因。结论上海地区志贺菌的耐药情况严重,且福氏和宋内志贺菌对部分抗菌药物的抗药性差别较大;福氏志贺菌毒力基因携带率高,毒力强,应注意防范。Objective To investigate the antibiotic resistance and the virulence genes of shigella in shanghai from 2010 to 2014. Methods Susceptibilities to 13 antibiotic agents were tested by Kirby-Bauer(K-B) in 139 shigella flexneri and 243 shigella sonnei strains. Meanwhile, four virulence genes were detected by polymerase chain reaction ( PCR). Re- sults Most of the shigella isolates were found to be resistant to nalidixic acid (97.4%), streptomycin (96. 1% ), ampi- eillin (84. 8% ), trimethoprim (77.7%) and sulphame-thoxazole/trimethoprim (77. 2% ). There were statistically signif- icant differences in the drug resistance to ampicillin, cefotaxime, cefmetazole, chloramphenicol, amoxicillin-clavulanic acid, tetracycline, ciprofloxacin, norfloxacin and levofloxacin between the two types of shigella ( all P 〈 0.05 ) , which were both highly resistant to multiple antibiotics. All of the 382 Sigella strains had the ipaH gene. 63.3% strains of shigella flexneri had all the five genes of ipall, ial, sen, set1 A and set 1B, while 50. 6% strains of shigella sonnei had three genes of ipaH, ial and sen. Conclusions The antibiotic resistance of the shigella strains in Shanghai is serious, but the resistance pat- terns of the shigella sonnei strains are significantly different from those of the shigella flexueri strains. The shigella flexneri strains have a high rate of virulence genes and we should pay attention.
