超声弹性成像应变率比值法对乳腺肿块良恶性鉴别诊断价值

摘　　要：目的探讨超声弹性成像应变率比值法(UE比值法)对乳腺肿块良恶性鉴别诊断价值。方法对72例乳腺肿块的女性患者做为观察对象,进行常规超声扫描(对照组),后进行UE扫描(观察组),对两组患者的乳腺肿块的良恶性诊断效果及其内部结构病变情况进行定期临床随访、统计实验检测数据分析影像学图像分析及记录详细临床信息。结果观察组中对乳腺肿块良性与恶性的诊断准确率显著高于对照组,差异具有统计学意义(t=3.624,P=0.001;t=4.313,P=0.001)。观察组在对乳腺肿块病变的良恶性诊断效果方面(灵敏度、特异度、阳性预测率、约登指数)更优于对照组,且两者差异均具有统计学意义(t依次为2.871、3.032、3.643、6.511,P依次为0.001、0.001、0.001、0.023)。此外UE扫描方式检查获得的影像学图像更为清晰准确,能为临床疾病诊断和治疗提供了良好依据。结论在对乳腺肿块良恶性鉴别诊断时,采取UE比值法检查具有快速简便、准确率高、分辨率高及特异性好的临床效果,具有较高的临床推广价值。Objective To explore the value of strain rate ratio by ultrasonic elastography （UE） in the differential diagnosis to the benign and malignant breast lumps, providing the effective basis of the clinical treatment for the benign and malignant breast lumps. Methods 72 patients with breast lumps who were received in our hospital from Jan. 2015 to Dec. 2015 were selected as the observation subjects. The patients under the premise of voluntary were examined by the conventional ultrasound scanning （control group） firstly and the UE scaning （observation group）. Then, we observed the clinical effect of diagnoed, detecting the internal structural lesions of breast lumps in two groups of patients with breast lumps, analyzed statisticaly the experimental data and the radiographic image, and recorded the detailed information. Results The diagnosis accuracy to the benign and malignant breast lumps in the observation group was significantly higher than that in the control group; and the difference was statistically significant （ t = 3. 624, P = 0. 001; t = 4. 313, P = 0. 001 ）. The observation group showed better effect in the diagnosis effect （sensitivity, Specific degrees, positive predictive rate, index） than that in the control group, and the differences were statistically significant （t were successively 2. 871,3. 032,3. 643,6. 511 ; Pwas successively 0.00, 0.01,0.00, 0.03） ; And the UE scan way could get more clear and accurate radiographic images than theconventional ultrasound scanning, providing a good basis for clinical disease diagnosisand treatment. Conclusion The UE ratio method has the reasonable and effective clinical effect, high accuracy, high resolution and rapid specificity in the diagnosis and treatment for the benign and malignancy breast lumps, which has the high clinical value.
