肛门直肠周围脓肿及肛瘘MRI诊断(附45例报告)

摘　　要：目的通过回顾性分析肛门直肠脓肿及肛瘘患者术前的MRI影像资料,总结不同系列对病灶显示的准确性,以及MRI检查具有达到分型诊断的临床价值。方法对2015年7月~2016年5月临床通过体检已诊断肛周感染性病变的患者45例,进行术前盆底MRI检查,术后回访临床资料,把影像资料与术后结果进行比对,分析影像诊断符合率。结果 45例肛周感染性病变中,影像诊断肛瘘42例,肛周脓肿3例;肛瘘MRI正确分型39例,无法分型1例;分型错误2例;其中正确分型中,括约肌间型肛瘘15例;经括约肌型肛瘘肛瘘19例;括约肌上型肛瘘5例;括约肌外型0例。结论高场强MRI可清晰显示肛周脓肿、肛瘘与肛周组织的关系,脓肿可准确定位,瘘管与肛门外括约肌的关系可达到分型诊断,对临床治疗具有指导性意义。Objective Through the anus rectum abscess and preoperative patients with anal fistula were retrospectively analyzed MRI image data, summarizes the different series of lesions showed accuracy, and the clinical value of MRI has reached classification diagnosis. Methods In July 2015-May 2016 clinical has passed a medical diagnosis crissum infectious disease pa tients, 45 cases of preoperative MRI of pelvic floor, postoperative clinical data review, the image data and compare the postoperative results, should be the imaging diagnostic coincidence rate. Results 45 cases of anus week in infectious disease, imaging in the diagnosis of anal fistula 42 cases, 3 cases of anus week abscess. MRI correct classification of 39 cases of anal fistula, can type in 1 case; Classification error in 2 cases ; The correct classification, type between the sphincter anal fistula 15 cases;By the sphincter anal fistula type anal fistula （19 cases） ; Type on the sphincter anal fistula in 5 cases; Sphincter 0 cases. Conclusion The high field strength MRI can clearly show crissum abscess, fistula and erissum organization relationship, abscess can accurate positioning, fistula and anal sphincter classification diagnosis can be obtained, the relationship between has guiding significance to the clinical treatment.
