结扎束血管闭合系统在阴式广泛子宫切除术中的应用研究

摘　　要：目的:评估新型结扎束血管闭合系统(简称LF4318)在阴式广泛全子宫切除中应用的临床疗效,探讨其临床价值。方法:回顾性分析本院2014年1月至2015年12月收治的早期宫颈癌(ⅠA2~ⅡA期)并行阴式广泛子宫切除+腹腔镜下盆腔淋巴结切除术83例患者,其中在阴式广泛子宫切除术中使用LF4318为观察组(38例),未使用LF4318为对照组(45例),记录并比较两组一般情况和手术相关指标。结果:两组年龄、体质量指数(BMI)、腹部手术史、FIGO临床分期、病理组织类型比较,差异均无统计学意义(P>0.05)。观察组手术时间(177.1±48.1分钟)明显少于对照组(226.2±70.5分钟),观察组术中出血量(159.0±77.4 ml)明显少于对照组(233.8±102.1 ml),差异均有统计学意义(P<0.01);两组输血、平均住院日、术后并发症及复发情况比较,差异均无统计学意义(P>0.05)。结论:LF4318应用于阴式广泛子宫切除术治疗早期宫颈癌,对宫旁组织和子宫血管进行闭合和切割更迅速、可靠,缩短手术时间,减少出血量,使手术更加高效,具有较好的临床应用价值。Objective： To evaluate the clinical effect and value of Ligasure Vessel Sealing System（ LF4318） in radical vaginal hysterectomy.Methods： A total of 83 patients with early-stage cervical cancer（ ⅠA2-ⅡA） undergoing radical vaginal hysterectomy and laparoscopic pelvic lymph node dissection in our hospital from January2014 to December 2015 were selected.The patients were divided into observation group（ using LF4318 group,n= 38） and control group（ conventional surgery group,n = 45）,and the general information and operation related index were compared.Results： No significant differences of the mean age,BMI,history of previous abdominal surgery,clinical stage of FIGO and the histological types were observed between two groups.The average duration of operation in observation group（ 177. 1±48. 1 min） was shorter than that in control group（ 226. 2±70. 5 min）,with statistically significant differences（ P〈0. 01）.The intra-operative bleeding volume in observation group（ 159. 0 ±77. 4 ml） was lower than that in control group（ 233. 8 ±102. 1 ml）,with significant differences（ P〈0. 01）.There were no significant differences of the rate of blood transfusion,the average duration of hospitalization and the incidence of complications between the two groups.Conclusions： The application of LF4318 in the radical vaginal hysterectomy can improve the vascular and tissue closure and cutting,shorten the operation duration,reduce the bleeding volume,to make the operation more effective,with clinical application value.
