静脉通路管理联合气压泵治疗对危重患者预防下肢深静脉血栓的效果评价

摘　　要：目的探讨静脉通路管理联合气压泵治疗仪(IPC)对危重患者预防下肢深静脉血栓形成(DVT)的效果。方法选择2015年1月1日—2016年1月31日入住重症监护室的160例存在DVT发生风险的患者,随机分为试验组及对照组,每组80例。对照组采用常规翻身、按摩、运动等预防下肢DVT的方法,试验组在此基础上采用静脉通路管理联合气压泵进行双下肢气压治疗,2组患者在治疗前后1周予血管多普勒超声检查双下肢深静脉血流情况。对2组患者肿痛和舒适度等进行观察,以及对栓子直径、检出率和DVT发生率进行比较。结果 2组患者均无DVT发生,试验组栓子检出率低于对照组(P<0.05),主要为可吸收性栓子(直径0.1~0.3 cm),通过静脉通路管理和气压泵治疗后在下肢消除肿胀、减轻疼痛及提高舒适度方面效果明显。结论静脉通路管理联合气压泵的综合治疗可有效的预防ICU危重患者下肢深静脉血栓形成。Objective To discuss the effect of venous access management combined with intermittent pneumatic com-pression （IPC） in preventing deep venous thrombosis （DVT） in critical patients. Methods 160 cases with the risk of DVT in ICU during the period from Jan., 1,2015 to Jan., 31,2016 were enrolled and randomly divided into 2 groups： experiment group and control group, 80 cases in each; conventional methods to prevent DVT, such as massage and exercise were applied to cases in both groups while venous access management combined with IPC was added to cases in experiment group; 1 week before and after treatment,all the cases in both groups received Doppler ultrasound examination of the blood flow of deep venae in lower limbs, an observation was made to the swelling pain and comfort degrees and a comparative study to the dia-meters of embolus, the detection rate and the occurrence of DVT between the 2 groups. Results No DVT occurred in both groups, the detection rate of embolus in experiment group was lower than that in control group （P 〈 0.05）, most embolus were self absorbable with the diameter of 0.1 -0.3 cm）; venous access management combined with intermittent pneumatic compres-sion provided obvious effect in relieving swelling pain and promoting comfort degrees. Conclusions Venous access manage-ment combined with intermittent pneumatic compression can effectively prevent the occurrence of DVT of lower limbs in criti- cal patients in ICU.
