清热活血方联合西药治疗慢性稳定性心绞痛20例

摘　　要：目的观察清热活血方联合常规西药治疗慢性稳定性心绞痛的临床疗效,并探讨其作用机制。方法将60例慢性稳定性心绞痛患者随机分为3组:西药对照组予常规西药治疗;活血中药组在常规西药的基础上加冠心Ⅱ号方;清热活血中药组在常规西药基础上加冠心Ⅱ号方及清热中药,每组20例。治疗4周后比较3组总有效率,治疗前后血浆C反应蛋白(CRP)、纤维蛋白原(FIB)水平。结果 3组心绞痛疗效总有效率比较差异无统计学意义(P>0.05)。清热活血中药组CRP明显下降,治疗前后差值较西药对照组及活血中药组有显著差异(P<0.01,P<0.05)。清热活血中药组FIB显著降低,治疗前后差值较西药对照组有显著差异(P<0.05),与活血中药组比较无统计学差异(P>0.05)。结论清热活血方联合常规西药治疗慢性稳定性心绞痛,能够有效抑制炎症反应,改善血液高凝状态,疗效优于常规西药及活血方联合常规西药治疗,值得临床推广、运用。Objective To investigate the curative effect of heat-clearing and blood-activating Chinese herbal medicine prescription combined with Western medicine in the treatment of chronic stable angina, and to explore the mechanism. Methods Sixty cases of chronic stable angina patients were randomized to the Western medicine control group treated with standardized Western medicine, the blood-activating and heat-clearing and blood-activating group treated with GuanxinⅡ formula or GuanxinⅡ formula and heat-clearing Chinese medicine respectively in addition to standardized Western medicine treatment, with 20 in each group. Three groups were ob-served four weeks. The total effective rate, CRP, FIB were detected. Results The total effective rate had no significant difference among three groups（P〈0.05）. The heat-clearing and blood-activating group showed superior effects in reducing the CRP, and there was signifi-cant difference as comparing with the Western medicine control group and the blood-activating group（P〈0.01 or P〈0.05）. The heatclearing and blood-activating group also showed superior effects in reducing the FIB, and there was significant difference as comparing with the Western medicine control group（P〈0.05）, but there was no significant difference as comparing with the blood-activating group（P〈0.05）. Conclusion For chronic stable angina patients, the heat-clearing and blood-activating Chinese herbal medicine prescription combined with Western medicine in the treatment of chronic stable angina can effectively inhibit the inflammatory reaction and improve the hypercoagulable state of blood, the curative effect is superior to Western medicine and heat-clearing and blood-activating Chinese herbal medicine prescription combined with Western medicine, and is worthy of clinical application.
