舒芬太尼复合不同剂量罗哌卡因对剖宫产产后镇痛的影响

摘　　要：目的探讨舒芬太尼复合不同剂量罗哌卡因对剖宫产产后镇痛的影响。方法将60例剖宫产产妇随机分为A组和B组,各30例。术后A组给予200 mg罗哌卡因+50μg舒芬太尼镇痛,B组给予250 mg罗哌卡因+50μg舒芬太尼镇痛。观察静息、运动状态下术后8 h、12 h、24 h、48 h时镇痛效果(VAS评分)及术后48 h内不良反应发生率。结果 B组静息及运动状态下术后8 h、12 h、24 h、48 h时VAS评分均低于A组(P<0.05或P<0.01);A组术后出现恶心2例(6.67%),B组出现1例(3.33%),两组不良反应发生率比较无统计学意义(P>0.05),均未出现呼吸抑制及尿潴留。结论剖宫产术后给予50μg舒芬太尼复合250 mg罗哌卡因镇痛效果明显优于50μg舒芬太尼复合200 mg罗哌卡因,不良反应未见增加,镇痛效应更佳。Objective To investigate the effects of sufentanil combined with different dose of ropivacaine for cesarean section postpartum analgesia. Methods Sixty cases of cesarean section,were randomly divided into group A and group B,each group of 30 cases of cesarean section under combined spinal epidural anesthesia. group A were given 200 mg ropivacaine added with 50 μg sufentanil and group B were given 250 mg ropivacaine added with 50 μg sufentanil for postoperative analgesia. Resting and motion state after 8 h,12 h,24 h,48 h analgesia（ VAS score）,postoperative incidence of adverse reactions in 48 h were observed. Results The resting and motion state after 8 h,12 h,24 h,48 h,the VAS scores in group A were lower than those in the group A,the difference was statistically significant（ P〈0.05 or P〈0.01）. Group A occurred in 2 cases of nausea,group B of 1 cases after operation,the incidence rate was6. 67% and 3. 33%,the difference was not statistically significant（ P〉0.05）,no respiratory depression and urinary retention occurred. Conclusion The analgesic effect of 50 μg sufentanil and ropivacaine 250 mg was significantly better than that of 50 μg sufentanil combined with ropivacaine 200 mg,no obvious adverse reactions increased in the two group.
