摘　　要：目的了解老年患者出院过渡期社会支持及社区资源利用情况。方法采用随机抽样的方法,以社会支持量表、老年人社区服务资源状况与卫生服务需求调查表为调查工具,对银川市某三甲医院住院,年龄≥60岁的老年人群在出院1个月、3个月、6个月三个时段进行调查,共收集有效问卷290份。结果老年患者出院后社会支持总分、主观支持、客观支持、对支持的利用度得分均较低,且在出院3个月时最低;有配偶者、夫妻同住者、曾经的职业是干部者社会支持得分均高于其他组别,经济来源是子女补贴者社会支持得分低于有退休金及政府补贴者;老年患者出院后主要以有血缘关系的家人照护为主,雇佣照护、钟点照护、家庭病床、访视护理等社区服务资源的利用率极低。结论社区应建立完善的老年人照料服务体系,为老年患者提供出院后可利用的医疗资源信息平台及途径,提高老年患者的生活质量。Objective To explore the social support and community resources utilization of elderly patients discharged from hospital in the transitional period.Methods 316 elderly patients in Yinchuan were selected by randomly sampling method and studied by cross-sectional study with social support scale and the elderly community service resource condition and the health ser- vice demand questionnaire.Results The score of the total social support, subjective support, objective support, utilization of support were all lower,and in the 3 months were minimum; Person who has a spouse,husband and wife live together,once the career is a cadre of social support score were higher than other groups, economic source provider in children scored lower on social support than have pension and government subsidies; Elderly patients after discharge mainly related family care,employment,the hour care, family care bed, visit the nursing community services such as resource utilization rate is extremely low.Conclusion The com- munity should establish perfect care service system of elderly, to provide elderly patients after discharge available medical resource information platform and the ways to improve the quality of life in elderly patients.
