摘 要:目的探讨C反应蛋白(CRP)水平与2型糖尿病病程早期症状性周围神经病变(DPN)的相关性。方法对我院103例糖尿病病程≤5年的2型糖尿病患者进行神经电生理检查,纳入其中49例DPN患者。对纳入病例进行多伦多临床评分系统(TCSS)评分,将有DPN症状、TCSS〉5分的患者归为症状性DPN组(ESDPN组,28例);将无DPN症状、TCSS≤5分患者归为无症状DPN组(NSDPN组,21例)。检测所有患者血清空腹C肽(FC-P)、2小时C肽(2hC-P)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2h血糖(2hPG)、血脂及CRP。结果各组间FC—P、2hC—P、HbA1c、FPG、2hPG、血脂比较差异均无统计学意义(P〉0.05);ESDPN组CRP水平显著高于NSDPN组[(7.22±5.63)mg/L比(1.46±0.80)mg/L,P〈0.05];CRP与TCSS评分有显著相关性(r=0.463,P=0.013);Logistic回归分析发现CRP水平与早期症状性周围神经病变的发生有关(OR=1.182,95%CI1.015—1.375)。结论CRP与2型糖尿病病程早期症状性DPN的严重程度相关,CRP可能参与了糖尿病病程早期症状性DPN麻木、刺痛感等症状的产生。Objective To evaluate the the relationship between C-reactive protein and the symptomatic diabetic peripheral neuropathy(DPN) in the early stage of type 2 diabetes. Methods One hundred and three patients of type 2 diabetes with the duration less than 5 years were enrolled. Forty nine DPN patients were diagnosed by neural electrophysiological test (NET). Toronto clinical scoring system (TCSS) was conducted in all these patients. Twenty eight symptomatic DPN patients with neural symptom diagnosed by TCSS were enrolled into SDPN group and 21 non-symptomatic DPN patients were enrolled into NSDPN group. The clinical data of patients, including triglyceride (TG), total cholesterolterol (TC), high-denity lipoproteincholesterol( HDL-C ), low-density tipoproteincholesterol ( LDL-C ), Hemoglobin Ale (HbAlc) ,fasting plasma glucose(FPG) ,2-hour postprandial plasma glucose(2h PG) ,fasting C peptide (FC-P) ,2-hour C peptide ( 2h C-P), C reactive protein (CRP) were recorded and analyzed. Results There was no significant difference in sex, smoking story, 2h PG, FC-P, TC, HDL-C, LDL-C, HbA1 c, FPG and TG, while the CRP was statistically different between the ESDPN group and the NSDPN group [ ( 7.22±5.63 ) mg/L vs ( 1.46 ± 0.80 ) mg/L, P 〈 0.05 ) ]. CRP level of ESDPN group correlated with TCSS( r = 0. 463, P = 0. 013 ). Logistic regressions analysis demonstrated that CRP level was associated with symptomatic DPN in the early stage of type 2 diabetes. Conclusion The symptomatic DPN in the early stage of diabetes is associated with increased CRP, especially associated with the severity of the symptoms of the diabetic neuropathy such as numbness and prickling.
