早期糖尿病肾病患者联合检测血清CysC与尿mALB/Cr的临床意义

摘　　要：目的探讨早期糖尿病肾病患者联合检测血清胱抑素C(CysC)与尿微量白蛋白-肌酐比值(mALB/Cr)的临床意义,为临床的早期发现和早期诊断提供实验依据。方法将190例糖尿病患者根据临床诊断分为单纯糖尿病(DM)组66例、早期糖尿病肾病(DN1)组63例和糖尿病肾病(DN2)组61例,并选取同期健康体检者63例作为健康对照组,分别采用胶乳增强免疫比浊法、免疫比浊法和肌氨酸氧化酶法检测血清CysC、尿微量白蛋白和尿肌酐的浓度,并对检测结果进行分析。结果 DN1组患者血清CysC和尿mALB/Cr浓度均高于DM组和健康对照组(P<0.05),DN2组患者血清CysC和尿mALB/Cr浓度均明显高于DN1组、DM组和健康对照组(P<0.05),且随着病情的进展血清CysC和尿mALB/Cr浓度相对增高;两项指标联合检测的真阳性率为88.89%,明显高于单项指标检测的真阳性率。结论血清CysC和尿mALB/Cr联合检测对早期糖尿病肾病有重要的临床意义。Objective To explore the clinical significance of early diabetic nephropathy patients of combined detection of serum cystatin C（Cys C）and urinary albumin/creatinine ratio（mAlb/Cr）and provide experimental basis for clinical early detection and early diagnosis.Methods 190 cases of diabetic patients according to clinical diagnosis for simple diabetes mellitus（DM）group（n=66）,early diabetic nephropathy（DN1）group（63cases）and diabetic nephropathy（DN2）group（61cases,and select corresponding health examination in 63 cases as healthy control group,respectively using latex particle enhanced immuno turbidimetric method,immune turbidity method and sarcosine oxidase method in detection of serum CysC,urine albumin and creatinine concentration ratio,and on the detection results analysis.Results DN1 group serum cystatin C and urinary mAlb/Cr concentration were higher than that of DM group and the healthy control group（P〈0.05）,DN2 group serum cystatin C and urinary mAlb/Cr concentration were significantly higher than those of the DN1 group and DM group and the healthy control group（P〈0.05）,and disease progress of serum cystatin C and urinary mAlb/Cr concentration was increased with;two indexes of combined detection of true positive rate was 88.89%,significantly higher than the detection of single index of the true positive.Conclusion Serum CysC and urinary mALB/Cr combined detection of early diabetic nephropathy has important clinical significance.
