颊脂垫瓣与异种脱细胞真皮基质修复颊部软组织缺损疗效观察

摘　　要：目的观察颊脂垫瓣（PBFPF）与异种脱细胞真皮基质（ADM）修复治疗口腔颊部黏膜缺损的t临床效果。方法2013年1月至2015年12Pl行颊部肿瘤切除术后颊黏膜缺损患者28例，随机分为颊脂垫转移皮瓣修复组和异种脱细胞真皮基质修复组，每组各14例，术后随访2周-6个月，并对术后伤口愈合情况、并发症、张口度、咀嚼功能等方面进行评价和追踪随访。结果颊脂垫瓣修复组总有效率85．71％，异种脱细胞真皮基质修复组总有效率92．85％，颊脂垫瓣修复组并发症发生率11．1l％，异种脱细胞真皮基质修复组并发症发生率5．55％，两组比较差异无统计学意义（P〉0．05）。结论两种方法均可满足颊部肿瘤切除术后黏膜缺损的修复，且均具有创伤小、并发症少和手术风险小等优点。Objective To observe clinical effect of restorative treatment of oral and buccal mucosa defect with pedicled buccal fat pad flap （ PBFPF ） and acellular dermal matrix （ ADM ） , Method From January 2013 to December 2015, 28 cases of patients with buccal tumor resection and buccal mucosa defect in Huludao Central Hospital were enrolled and randomly divided into one group with repair of pedicled buccal fat pad flap and another group with repair of acellular dermal matrix, 14 cases for each group. After the surgery, they were visited randomly for 2 weeks to 6 months, evaluate and follow up were made on the postoperative wound healing, complications, mouth opening, and masticatory function etc. Result Total effective rate of group of pedicled buccal fat pad flap repair was 85.71% （ 12/14 ） , while that of group of acellular dermal matrix repair was 92.85% （ 13/14 ） . Occurrence rate of complication of pedicled buccal fat pad flap repair was 11.11% （ 2/18 ） , while that of acellular dermal matrix repair was 5.55% （ 1/18 ） , and the difference was not statistically significant （ P〉0.05 ） . Conclusions Both methods can be used to repair the post resection mucosal defect of buccal tumor and have the advantages of less trauma, less complications, less risk of operation and so on.
