摘　　要：目的探讨布鲁氏菌性脊柱炎外科手术治疗效果。方法回顾性分析解放军第五医院骨科自2011年2月至2015年2月采用外科手术治疗的23例布鲁氏菌性脊柱炎患者的临床资料,包括临床疗效、影像学特点、实验室检查及JOA评分改善情况。结果本组23例患者,手术时间60~150 min,平均90 min;术中出血300~800 ml,平均450 ml;无手术并发症;出院后随访3~46个月,平均25个月。23例患者中,仅3例残余腰部疼痛,其余症状均消失;术后2个月,JOA评分改善率为65%;术后6个月,红细胞沉降率及C反应蛋白均正常;术后1年,患处均骨性愈合。结论掌握手术适应证的情况下,对布鲁氏菌性脊柱炎患者进行手术治疗,可以取得满意的临床效果。Objective To explore the efficacy for the surgical treatment of brucellar spondylitis. Methods A retrospective study was performed on 23 patients with brucellar spondylitis underwent the surgical treatment from February 2011 to February 2015. Clinical data were collected and the clinical presentation,physical symptom,radiological features and laboratory findings as well as JOA score were reviewed during the course. Results The average operating time was ranged from 60-150 minutes,mean 90 minutes; the intraoperative blood loss was ranged from 300-800 m1,mean 450 ml; no one had complication symptom during the surgery. Patients were followed up for 3-46 months,mean 25 months. Among 23 cases,only 3 cases had minor residual pain on low back,the other symptoms were disappeared. Two months after surgery,the improvement of JOA was 65%; 6 months after surgery,erythrocyte sedimentation rate and c-reactive protein were normal; 1 year after surgery,the lesion site were bone healing in all cases. Conclusion Surgical treatment for patients with brucellosis spondylitis is safe and effective when meeting the indications.
