不同类型突发性耳聋的临床疗效观察

摘　　要：目的探讨不同类型的突发性耳聋的临床疗效。方法选取2013年2月至2015年6月我院收治的突发性耳聋120例,根据突发性耳聋的不同类型分为A、B、C三组,A组患者为高频听力下降,B组为低频听力下降,C组为全频听力下降,每组40例,三组患者均采用大剂量照射急速联合前列地尔治疗,观察A、B、C三组患者的临床治疗效果。结果 A组患者中痊愈4耳,显效6耳,有效10耳,治疗总有效率41.7%;B组痊愈22耳,显效13耳,有效11耳,治疗总有效率82.1%;C组痊愈7耳,显效9耳,有效13耳,治疗总有效率55.8%。B组患者的总有效率明显高于A、C两组,三组患者组间差异明显,有统计学意义(P<0.05),可见大剂量激素联合前列地尔对于治疗低频率下降性突发性耳聋的疗效更为显著。结论采用大剂量激素联合前列地尔对于低频率下降的突发性耳聋的临床疗效明显高于高频率下降和全频率下降的突发性耳聋患者,该方法能够有效缓解耳聋、耳鸣、目眩等症状,该方法有助于根据突发性耳聋类型来判断预后效果。Objective To investigate the clinical effect of different types of sudden deafness. Methods A total of 120 cases of sudden deafness in our hospital from February 2013 to June 2015,according to the different types of sudden deafness into A, B, C three groups, patients in group A were decreased for high frequency hearing, group B for low-frequency hearing decreased, group C for all frequency hearing decreased, with 40 cases in each group, three groups of patients were treated with large dose of radiation rapidly combined with alprostadil in the treatment of observed A, B, C three groups of patients with clinical curative effect. Results The patients in group A were recovered in 4 ears, 6 ears were markedly effective, effective ear treatment, the total efficiency was 41.7%;group B patients recovered 22 ears, 13 ears were markedly effective, effective 11 ears treatment, the total efficiency was 82.1%;patients in group C in 7 ears were cured, 9 ears were markedly effective, effective in 13 ears treatment, the total efficiency was 55.8%.Total efficiency of patients in group B were significantly higher than those in group A and C, three groups of patients between groups difference was statistically significant（P〈0.05） and visible large dose hormone combined with Alprostadil for the treatment of low frequency rate of decline in the curative effect of sudden deafness is more significant. Conclusion Using high dose methylprednisolone combined with Alprostadil for the clinical efficacy of low frequency decreased the rate of sudden deafness was significantly higher than that of high frequency decreased and decreased the frequency of sudden deafness patients. The method can effectively alleviate deafness, tinnitus, dizzy wait for a symptom, the method helps to judge P prognosis according to the types of sudden deafness.
