活血通络膏联合低剂量利伐沙班预防全髋关节置换术后深静脉血栓形成的临床研究

摘　　要：目的:探讨活血通络膏联合低剂量利伐沙班预防全髋关节置换术(THA)术后并发下肢深静脉血栓形成(DVT)的有效性及安全性。方法:将72例接受单侧THA手术的患者随机分为试验组、对照组,每组各36例。试验组予低剂量利伐沙班片口服,联合活血通络膏外敷,对照组予正常剂量利伐沙班片口服,连续2周。观察两组患者凝血指标、大腿及小腿周径差值、DVT形成情况及用药安全情况。结果:1)试验组和对照组术后APTT,PT均明显延长(P<0.05),D-D、患肢大腿及小腿周径差值均明显降低(P<0.05);2)两组间比较,差异均无统计学意义(P>0.05);3)两组患者HGB、术中出血量、输血量及术后引流量比较,差异均无统计学意义(P>0.05);4)试验组下肢深静脉血栓发生率为8.3%(3/36),对照组为2.8%(1/36),差异无统计学意义(P>0.05);5)两组患者治疗期间均无消化道出血、皮肤黏膜出血、胃肠系统异常等不良反应发生,肝、肾功能检查均未见明显异常。结论:应用活血通络膏联合低剂量利伐沙班预防THA术后DVT,能减少利伐沙班用量、降低DVT发生率,有效性和安全性与正常剂量利伐沙班相当。Objective：To observe the effectiveness and safety of application of Huoxuetongluo ointment combing low dose of Rivaroxaban in preventing deep venous thrombosis（DVT）after total hip arthroplasty（THA）.Methods：72patients underwent unilateral THA were randomly divided into 2groups,36 cases in each group.The patients in the treatment group were treated with external application of Huoxuetongluo ointment combing oral application of low dose of Rivaroxaban,while the others in control group were treated with normal dose of Rivaroxaban orally.Results：In both groups,PT and APTT were significantly prolonged（P〈0.05）,and D-D,leg thigh and calf circumference difference were significantly reduced（P〈0.05）.There was no significant difference between the 2groups（P〉0.05）.There was no statistical difference in HGB,intraoperative blood loss,total transfusions and postoperative drainage between the two groups（P〉0.05）.The incidence rate of lower deep vein thrombosis in treatment group was 8.3%（3/36）and control group 2.8%（1/36）,and the difference was not statistically significant（P〉0.05）.No adverse effect was observed in both groups such as alimentary tract hemorrhage,skin and mucosa hemorrhage,alimentary tract indisposition,hepatic disfunction and renal disfunction.Conclusion：Application of Huoxuetongluo ointment combing low dose of Rivaroxaban in preventing DVT after THA can reduce the dosage of Rivaroxaban and reduce the incidence of DVT.The effectiveness and safety are comparable to the normal dose of Shaaban.
