手指全形再造临床应用效果的对比研究

摘　　要：目的观察手指全形再造的手术过程和手术效果,并与传统手指再造方法进行比较。方法自2010年1月至2016年12月,对52例手指缺损患者进行手指再造。其中,采用传统方法进行手指再造40例,采用全形再造法进行手指再造12例。并对手术过程和术后效果进行比较分析,观察指标包括手术时间、手术创伤、手术风险、术后并发症、再造手指的感觉功能、再造手指的运动功能、再造手指的外形、足趾供区的外形与功能、患者的满意度等。结果全形再造法手指再造可以达良好的术后外形,外形满意度高于传统手指再造法。再造手指的感觉和运动功能方面两种方法无明显差异。同时,全形再造法手术时间较长、创伤较大、手术风险较高。结论手指全形再造可以达到良好的手指美容修复效果,适用于对外形要求较高的患者。同时,存在手术难度大和手术风险高等缺点,需要在临床上谨慎开展。Objective To evaluate the effect of full-shaped finger reconstruction compared with the traditional method. Methods From January 2010 to December 2016, 52 patients with finger defects received reconstruction in the Department of Plastic Surgery. Among them, the traditional method was performed in 40 cases and the full-shaped reconstruction method was performed in 12 cases. Comparingthe surgical procedure and postoperative effect of both methods, the operative time, trauma and risk, postoperative com- plications, sensory, motion, and appearance of the reconstructed finger, contour and function of the donor toe, and patients＇ satisfaction were evaluated. Results Good postoperative appearance can be obtained through full-shaped finger reconstruction.The level of satis- faction is greater than with the traditional method. There is no significant difference of sensation and motion of the reconstructed finger between both methods. Better appearance, longer operation time, more trauma and higher risk emerged in the full-shaped reconstruction. Conclusion Good cosmetic effect can be achieved through finger full-shaped reconstruction. It is suitable for patients when there isa high expectation of postoperative appearance. The full-shaped fingerreconstruction method is more difficult and presents higher risk. Clinical promotion should hemade with caution.
