早产儿呼吸暂停应用枸橼酸咖啡因的疗效及对血内皮素、一氧化氮与β-内啡肽的影响

摘　　要：目的 综合分析早产儿呼吸暂停应用枸橼酸咖啡因的临床疗效及对血内皮素、一氧化氮与β-内啡肽的影响。方法 选取在118例呼吸暂停早产患儿临床资料作为研究对象,按照不同的治疗方法分为实验组与对照组,每组均为59例。对照组应用氨茶碱治疗,实验组应用枸橼酸咖啡因治疗。治疗后分析2组患者的临床治疗总有效率、临床症状变化情况以及对血内皮素、一氧化氮与β-内啡肽的影响等参数指标。结果 实验组治疗总有效率为84.75%（50/59）,对照组治疗总有效率为69.49%（41/59）,实验组治疗总有效率显著高于对照组（P〈0.05）。实验组腹胀发生率、心率加快发生率、喂养不耐受发生率、支气管肺发育不良发生率、烦躁不安发生率显著低于对照组（P〈0.05）。治疗后,实验组患儿的血内皮素、一氧化氮水平显著高于对照组（P〈0.05）,实验组患儿β-内啡肽水平显著低于对照组（P〈0.05）。结论 早产儿呼吸暂停枸橼酸咖啡因用于早产儿呼吸暂停的临床疗效显著,能够降低不良反应发生率。Objective To analyze the clinical effect of caffeine citrate on apnea and its effect on endothelin, nitric oxide and p-endorphin in premature infants. Methods 118 children with apnea-hypoprenalia were selected and divided into experimental group and control group according to different treatment methods. The control group were treated with aminophylline,and the experimental group were treated with caffeine citrate. The clinical symptoms, endotheliolysin, nitric oxide and p-endorphin in two groups of patients were compared. Results The total effective rate of 84.75% （50/59） in the experimental group was higher than 69.49% （41/59） in the control group （P〈0.05） . The incidence o f abdominal distension, heart rate, feeding intolerance,bronchopulmonary dysplasia and irritabHity were significantly lower in the experimental group than those in the control group （P 〈0.05）. After treatment, the levels of endotheliolysin, nitric oxide in the experimental group were significantly higher than those in the control group （P 〈0.05）, and the level of p endorphin in the experimental group was significantly lower than that in the control group （ P 〈0.05）. Conclusion Caffeine citrate has a significant clinical effect on apnea in premature infants, and can reduce the incidence of adverse reactions.
