有限切开复位钛缆环扎结合髓内钉固定治疗股骨转子下骨折

摘　　要：目的探讨采用有限切开复位钛缆环扎结合髓内钉固定治疗股骨转子下骨折的临床疗效。方法对20例股骨转子下骨折患者采用有限切开复位、钛缆环扎固定骨折端,自大转子顶点顺行插入股骨头状髓内钉进行固定。结果患者均获得随访,时间12~36个月。骨折均愈合,时间为3~6个月。按照Beigessner标准评定骨折复位质量:解剖复位17例,轻度畸形复位3例。末次随访时按照Harris评分标准评定患髋功能:优14例,良4例,可2例,优良率为18/20。术后无感染、复位丢失、内固定失效等并发症发生。结论有限切开复位钛缆环扎结合髓内钉固定是治疗长斜形和长螺旋形股骨转子下骨折合理而有效的方法,临床疗效满意。Objective To discuss the clinical and radiological results of using limited open reduction and internal fixation with cerclage cables and intramedullary nails for subtrochanteric fractures. Methods The 20 cases of subtrochanteric fractures were performed with limited open reduction and internal fixation with cerclage cables and intramedullary nails. Results All the patients were followed up for 12 - 36 months,and fracture healing time was 3 - 6 months.According to Beigessner＇s standards of fracture reduction,17 cases were anatomic reduction,3 cases were minor malreduction. At last follow-up,according to Harris hip score system,14 cases were excellent,4 cases were good,2 cases were acceptable,the excellent and good rate was 18 /20. There were no infection,reduction loss,implant failure cases.Conclusions In the treatment of long oblique and spiral fractures in the subtrochanteric femur,limited open reduction and internal fixation with cerclage cables and intramedullary nails are feasible and effective choice.
