摘 要:目的检测慢性乙型肝炎(CHB)患者外周血单个核细胞(PBMCs)中T细胞受体重排删除环(TRECs)的含量,进而评价CHB患者的胸腺近期输出功能,并分析TRECs含量与HBV病毒含量间的关系。方法利用实时荧光定量PCR法检测205例CHB患者PBMCs中TRECs含量及血清HBV DNA含量,同时选取32例健康人群作为对照组。结果患者组外周淋巴细胞中TRECs含量明显低于对照组,差异有统计学意义(P<0.05)。根据HBV DNA含量的不同分为HBV DNA阴性组、低病毒组(<1×105IU/ml)和高病毒组(≥1×105IU/ml),3组患者TRECs含量差异无统计学意义(t=2.58,P>0.05)。采用直线回归相关性分析发现,HBV DNA含量与TRECs含量之间无相关性(r=0.15,P>0.05)。结论 CHB患者PBMCs中TRECs含量低于正常人群,提示CHB患者胸腺近期输出功能明显降低;TRECs含量与HBV病毒含量间无明显相关性。Objective To analyze the content of T- cell receptor excision circles( TRECs) within peripheral blood mononuclear cells of chronic hepatitis B( CHB) patients to value the thymus output function of CHB,and to investigate the relation between thymus output function and HBV DNA in CHB patients. Methods Real- time fluorescence quantitative PCR was adopted to detect the contents of TRECs and serum HBV DNA in peripheral blood mononuclear cells( PBMCs) from 205 cases with CHB,and meanwhile,32 normal individuals were collected as the control group. Results The content of TRECs in peripheral blood of CHB patients was lower than that in the control group,and the difference was statistically significant( P〈0. 05). According to the difference of HBV DNA content,the results were divided into HBV DNA negative group,low virus group( 1 × 10^5 IU / ml)and high virus group,and the differences of TRECs contents among the 3 groups were not statistically significant( t = 2. 58,P〉0.05). Linear regression analysis showed that there was no correlation between HBV DNA content and TRECs content( r =0. 15,P〉0.05). Conclusion TRECs levels in CHB are lower than that in the healthy suggesting that the thymic output function of CHB patients significantly reduce,and there was no significant correlation between TRECs content and the content of HBV virus,and no significant correlation in HBV DNA.
