维持性血液透析患者残余尿量对钙磷代谢的影响

摘　　要：目的研究维持性血液透析(MHD)患者钙、磷代谢紊乱与残余尿量的关系,以便更好地保护残余尿量,控制钙、磷代谢紊乱。方法以血液透析中心MHD≥3个月患者作为研究对象。入选患者检测透析前全段甲状旁腺激素(i PTH)水平、血清钙磷水平。根据残余尿量(无尿组尿量≤100 m L/24 h和有尿组尿量﹥100 m L/24h)进行分组,对各组间血清钙磷与i PTH等进行比较。结果有尿组和无尿组i PTH比较差异有统计学意义(P<0.05),但血清钙磷差异无统计学意义(P>0.05)。结论维持性血液透析患者残余尿量对预防和减轻钙磷代谢紊乱、继发性甲状旁腺功能亢进有重要意义。Objective To study the relationship between the calcium - phosphorus metabolic disorders of MHD patients withand the residual urine in the Maintenance Hemodialysis （MHD） patients volume, in order to better protect the residual urine volume and control the calcium - phosphorus metabolic disorders. Method It tookThe patients who had receivinged MHD for more than ≥3 months in the hemodialysis center of the Shizuishan 1st People＇s Hospital were collected and te as the research object. It testeds the iPTH levels of , iPTH , serum calcium and phosphorus level in serum of the patients before receiving dialysis. Then their classifies patients were classified into different groups in accordance with the residual urine volume （urinefree group： urine volume ≤ 100 mL/24 h;urine group： urine volume 〉 100 mL/24 h） and compareds the groups in terms of their serum the calcium,phosphorus and iPTH in the groups. Results The iPTH difference of iPTH between urine - free group and urine group hasd statistics significance （ P 〈 0.05 ） while the differences between them in terms of the serum calcium and phosphorus were not do not have statistics significance. Conclusion Maintaining the residual urine volume of the maintenance hemodialysis patients has great significances on preventing and alleviating the calci- umphosphorous disorders and the secondary hyperparathyroidism.
