活血化瘀散结法治疗Ⅲ期糖尿病肾病气阴两虚夹瘀证临床研究

摘　　要：目的:观察活血化瘀散结法治疗Ⅲ期糖尿病肾病气阴两虚夹瘀证的临床疗效。方法:68例糖尿病肾病Ⅲ期气阴两虚夹瘀证老年患者随机平均分为观察组和对照组。对照组给予降低血糖、降低血压等常规疗法,观察组在对照组治疗基础上给予活血化瘀散结中药。检测两组患者治疗前后血糖、胰岛素及肾代谢功能指标,同时比较两组患者临床疗效及并发症。结果:治疗后,对照组患者临床有效率为70.59%,观察组临床有效率为91.18%,两组有效率比较,差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后血糖、胰岛素功能及肾脏代谢功能均发生变化;观察组治疗后血糖及部分胰岛素功能低于对照组,HOMA-β等胰岛素功能高于对照组,观察组患者治疗后血尿素氮、血肌酐、24小时尿微量白蛋白定量低于对照组,肾小球滤过率高于对照组,差异均有统计学意义(P<0.05)。结论:活血化瘀散结中药治疗Ⅲ期糖尿病肾病气阴两虚夹瘀证疗效确切,能够有效降低尿微量白蛋白,缓解肾衰竭进展。Objective：To investigate the effect of activating blood circulation to dissipate blood stasis in the treatment of patients with diabetic nephropathy III stage Qi Yin deficiency with blood stasis syndrome. Methods：68 cases with diabetic nephropathy m stage Qi Yin deficiency with blood stasis type were selected and divided into 2 groups,34 cases in the control group were treated with hypoglycemic, antihypertensive treatment,34 cases in the experiment group received more with activating blood circulation to dissipate blood stasis, blood glucose, insulin function and renal function index, clinical efficacy and complications were compared after the treatment. Results ：The clinical effect rate in the control group（70.59% ） was lower than the experiment group（91.18%, P 〈 0. 05）, compared with before treatment, blood glucose, insulin function and renal function in 2 groups changed, and compared with control group ,blood glucose and insulin function in the experiment group were lower, HOMA-β was higher, blood urea nitrogen, serum creatinine ,24 hours urine micro albumin quantity were lower, glomerular filtration rate was higher（ P 〈 0.05 ）. Conclusion ： Ac- tivating blood circulation to dissipate blood stasis in the treatment of patients with diabetic nephropathy Ⅲ stage Qi Yin deficiency with blood stasis type has definite therapeutic effect, which can effectively reduce urinary albumin, ease the progress of renal funetion.
