注射用益气复脉联合重组人脑钠肽治疗老年冠心病慢性心力衰竭急性加重期效果观察

摘　　要：目的:观察注射用益气复脉(冻干)联合重组人脑钠肽(rh BNP)治疗老年冠心病慢性心力衰(CHF)急性加重期疗效。方法:老年冠心病慢性心力衰竭急性加重期患者90例,按照随机数字表法分为对照组45例和治疗组45例。两组均在标准抗心衰治疗的基础上,对照组给予静脉滴注0.9%氯化钠注射液250 m L+注射用益气复脉(冻干)5.2 g,1次/d,连用7 d。治疗组给予静脉滴注0.9%氯化钠注射液250 m L+注射用益气复脉(冻干)5.2 g,1次/d,同时联合静脉滴注rh BNP,负荷量1.5μg/kg,之后以0.01μg/(kg·min)速度连续输注7 d。观察两组的临床治疗效果。结果:治疗组总有效率为88.9%,高于对照组的62.2%(P<0.05)。两组患者治疗后LVEDD、LVESD及LVEF水平对比治疗前均改善,治疗组较对照组改善情况更显著,差异均有统计学意义(P<0.05)。治疗后两组患者血浆NT-pro BNP、Cys C、hs-CRP水平较治疗前显著降低(P<0.05),治疗组均低于对照组(P<0.05)。结论:注射用益气复脉(冻干)联合重组人脑钠肽治疗老年冠心病慢性心力衰竭急性加重期临床疗效显著,可有效改善患者的心功能,降低其血浆NT-pro BNP、Cys C、hs-CRP水平。Objective： To observe the curative effect of Yiqifumai Injection combined with recombinant human Brain Natriuretic Peptide （ rhBNP ） in the treatment of elderly coronary heart disease patients with acute exacerbation of chronic heart failure （ CHF ） .Method： A total of 90 elderly coronary heart disease patients with acute exacerbation of CHF were enrolled and divided into the control group （ n=45 ） and the treatment group （ n=45 ） .On the basis of standard anti-heart failure therapy.The control group was given Yiqifumai Injection 5.2 g by intravenous drip, once daily, with 1 week.The treatment group was given Yiqifumai Injection 5.2 g by intravenous drip, once daily, and combined with rhBNP loading dose 1.5 Ix g/kg, then 0.01 p. g/（ kg ~ rain ） by continuous intravenous drip l with 1 week.The curative effects of two groups were observed.Result： The total effective rate of the treatment group was higher than that of control group （ 88.9% vs 62.2%, P〈0.05 ） . Left ventricular end-diastolic diameter （ LVEDD ）, left ventricular end-systolic diameter （ LVESD ） and left ventrieular ejection fraction （ LVEF ） after treatment improved in two groups compared to those before treatment, and the improvement in the treatment group were better than those in the control group, there were significant difference （ P〈0.05 ） .The plasma levels of NT-proBNP, CysC, hs-CRP were significantly lower after treatment in two groups, and the levels of these indices of the treatment group were lower than those of the control group （P〈0.05） .Conclusion： Yiqifumai Injection combined with rhBNP obtains an obvious efficacy for the treatment of elderly coronary heart disease patients with acute exacerbation of CHF.h can effectively improve the cardiac function and reduce the plasma levels of NT-proBNP, CysC, hs-CRP.
