四君子汤联合乌司他丁治疗重度烧伤临床研究

摘　　要：目的:观察四君子汤联合乌司他丁注射液治疗重度烧伤患者的临床疗效及对血清炎症因子及补体C3、C4、B因子的影响。方法:选取重度烧伤患者96例,按随机数字表法分组,对照组患者48例予以常规治疗,研究组患者48例予以四君子汤和乌司他丁注射液治疗,采肘静脉血测定两组治疗前后血清炎症因子及补体C3、C4、B因子,检测血常规状况,对比临床疗效及不良反应。结果:对照组有效率为72.92%,研究组有效率为89.58%,两组比较,差异有统计学意义(P<0.05);与对照组比较,研究组治疗后血清肿瘤坏死因子、白细胞介素-6、白细胞、中性粒细胞水平降低,血清白细胞介素-2、白细胞介素-10、补体C3、C4、B因子及血小板水平升高,差异均具有统计学意义(P<0.05);两组患者不良反应多为轻中度,患者能够耐受,对症处理后可缓解。结论:四君子汤联合乌司他丁注射液治疗重度烧伤患者疗效确切,能显著提高补体水平,降低炎症指标。Objective ： To investigate the clinical efficacy of Sijunzi Decoction combined with Ulinastatin in treatment of severe bums and the effect on the serum complement C3, C4 and B factor. Methods ： A total of 96 cases of patients with severe burns in our hos- pital were selected and randomly divided into 2 groups, with 48 cases in each group. The control group received conventional thera- py, and the treatment group was treated with Sijunzi Decoction combined with Ulinastatin. Serum inflammatory cytokines, comple- ment C3, CA, B factor, blood routine, the clinical efficacy and adverse reactions of both groups were compared after the treatment. Results：The effective rate （72.92%） of the control group was significantly lower than that （89.58%）of the treatment group （P 〈 O. 05）. Compared with the control group, the serum TNF and IL-6 levels were lower, serum IL-2 and IL-10 were higher, comple- ment C3, CA ,factor B were higher, with significant differences （P 〈 0.05 ）. The white blood cell and neutrophil levels decreased, platelet levels increased, with statistical significance（P 〈 O. 05）. The adverse reactions of the two groups were mild or moderate, which could be tolerated and easied with symptomatic treatment. Conclusion：Sijunzi Decoction combined with Ulinastatin in treat- ment of severe burns is effective and safety, in which the level of complement can be improved and the inflammation can be re- duced.
