摘  要:针刺通过干预丝裂原活化蛋白激酶信号转导通路(mitogen-activated protein kinase,MAPK)信号通路表达治疗脑缺血再灌注损伤机制的相关研究已逐渐开展并初见成效,但仍然存在一些亟待解决的问题,主要表现在:1各研究的造模方式不一,术后评价标准有待进一步考量;2治疗组一般为电针或头针,干预方法较为单调,且取穴、电针参数、观察时间参差不齐,没有统一的标准,具有一定的随意性;3各个研究中观察指标定性定量的检测方法不尽相同,横向可比性不大,而且采用的检测方法也比较传统,今后的实验可以适当创新性采纳先进的技术及设备;4现阶段实验研究对象较局限,绝大部分为各类大鼠,是否能与人体临床实际相一致也有待于将来大量的研究来印证;5现阶段的研究多是针对某单一的MAPK信号通路的家族成员进行观察,而众所周知MAPK家族相当庞大,并且仍然不断有新成员加入,加之生物体内的各种生化反应更是复杂繁琐,将来进一步的实验研究在设计时或许可以考虑同时观察两条、三条甚至更多,以便了解其之间的相互关系,也许针刺正是通过调整各通路之间的动态平衡而发挥对脑缺血再灌注损伤的治疗作用;6当前对于针刺通过干预MAPK信号通路表达治疗脑缺血再灌注损伤机制的相关研究仍然较多地停留在简单的验证及合理的推测上,尤其是对于研究相对较多的细胞外信号调节蛋白激酶信号通路(extracellular signal-regulated kinase,ERK)信号通路的作用至今仍然存在很大争议,这无疑给后续的研究埋下了路障。所以,在该研究领域一些根源性问题上达到共识的要求迫在眉睫,而怎样设计实验建立权威以及更深层次地阐明该机制是对我们提出的一项严峻而意义重大的挑战。Researches on the mechanism of acupuncture to treat for CIR by intervention of MAPK signaling pathway has been car- ried out and scientific results have been achieved. However, there are still some existing problems to be solved:(1)The modeling methods in these studies are various and the postoperative evaluation criteria need further consideration ; (2)The treatment group is generally electro-acupuncture or scalpel and the intervention method is monotonous. The choosing of acupoints, the parameters of e- lectro-acupuncture and the observation time were determined without a uniform standard. (3)The qualitative and quantitative detection methods of various indicators in observation are not the same and there was no adequate horizontal comparability between one another. The detection method were out-of-date and appropriate and innovative use of advanced technology and equipment in future experiments should be realized; (4) The experimental subjects are much limited, with the vast majority of the kinds are rats. Wheth- er the clinical effect on human is consistent with that of rats is to be confirmed in the future;(5) At present, many studies have fo- cused on the family members of a single MAPK signaling pathway. However, the MAPK family is rather large in size and some members of the family have not been explored. In addition, now that various biochemical reactions in organisms are complicatedand complicated ,further experimental studies should explore two, three or more at the same time to understand the relationship be- tween them. Perhaps the effect of acupuncture is achieved just by adjusting the dynamic balance between the various pathways in treatment of cerebral ischemia reperfusion injury;(~) The current researches on the mechanism of acupuncture through the interven- tion of MAPK signaling pathway in treatment of cerebral ischemia-reperfusion injury still remains in the level of simple validation and reasonable speculation.
