经胸超声心动图全程引导下房间隔缺损封堵术的临床应用

摘　　要：目的:探讨经胸超声心动图在房间隔缺损封堵术(transcatheter atrial septal defect,TCASD)的临床应用价值。方法:选取160例房间隔缺损(atrial septal defect,ASD)患者,随机分为观察组[经胸二维超声(transthoracic echocardiography,TTE)+胸实时三维超声心动图(real time three dimensional echocardiography,RT-3DE)]和对照组(X线+TTE)各80例,对比两组手术效果。结果:观察组手术时间和房间隔缺损封堵器释放次数均多于对照组(P<0.05),两组手术成功率、房间隔缺损封堵器一次释放成功率、房间隔缺损封堵器直径和残余分流比较差异无统计学意义(P>0.05);ASD最长径<20 mm时,观察组D值小于对照组(P<0.05)。ASD最长径≥20 mm且≤25 mm时,两组D值差异无统计学意义(P>0.05)。结论:TTE联合RT-3DE全程引导下行TCASD与X线引导下效果相当,可作为TCASD的引导和监测手段。Objective： To discuss the clinical value of transthoracic echocardiography（ TTE） in transcatheter atrial septal defect（ TCASD）.Methods： 160 cases of patients with atrial septal defect（ ASD） were selected and randomly divided into observation group（ TTE ＋ RT- 3DE） and control group（ X- ray ＋ TTE）,80 cases in each group. The surgery effects of the two groups were compared. Results： The operation time and ASD plugging device releasing time in observation group were significantly higher than those in control group（ P〈0. 05）. There were no significant differences in success rate,success rate of ASD plugging device releasing,ASD plugging device diameter and residual shunt in the two groups（ P〈0. 05）. D value in observation group was less than that in control group（ P〈0. 05） with the longest diameter of ASD 20 mm. There was no significant difference in D value between the two groups（ P〈0. 05） with the longest diameter of ASD ≥20 mm and ≤25 mm. Conclusion：TCASD under the guidance of the whole process of TTE combined with RT- 3DE has the same effect as that under the guidance of X- ray. TTE combined with RT- 3DE can be used as a guide and monitoring method of TCASD.
