不同浓度臭氧关节腔注射对膝骨性关节炎患者IL-1和IL-6的影响

摘　　要：目的观察不同浓度(20μg/m L、35μg/m L、50μg/m L)臭氧关节腔注射对膝骨性关节炎的临床疗效和炎症因子的影响,筛选出适合临床应用的最佳浓度,为膝骨性关节炎提供更加经济安全、简便有效的治疗方法。方法 90例膝骨性关节炎患者随机分为3组,分别采用20μg/m L、35μg/m L和50μg/m L浓度臭氧关节腔注射,治疗膝骨性关节炎。每周1次,总疗程为5周。结果 35μg/m L浓度臭氧关节腔注射治疗膝骨性关节炎的总有效率明显高于其他两组(P<0.05);35μg/m L和50μg/m L浓度臭氧关节腔注射后的KOA患者关节腔滑液中IL-1和IL-6含量明显低于20μg/m L浓度臭氧注射组(P<0.05),且35μg/m L较50μg/m L的降低幅度更大;不同浓度臭氧关节腔注射后,KOA患者血清IL-1和IL-6含量均有不同程度的降低,但结果缺乏统计学意义(P>0.05)。结论不同浓度臭氧关节腔注射对膝骨性关节炎均具有治疗作用,但35μg/m L在改善临床症状和降低炎症因子水平方面的作用更佳,且安全性较高,适合临床应用和推广。Objective To observe the effects of intra-articular injection with different concentrations（ 20μg / m L,35μg / m L,50μg / m L） of ozone on IL-1 and IL-6 in treating knee osteoarthritis（ KOA）,to screen for the most appropriate concentration and providing safer,more economic,convenient and effective therapy for treatment of KOA. Methods 90 patients with KOA were randomly divided into 3 groups,respectively administered with 20μg / m L,35μg / m L and 50μg / m L ozone injected into arthrosis. The treatment was applied once per week with a total of 5 weeks. Results Effective rate in 35μg / m L concentration group was significantly higher than that in the other two groups（ P〈0. 05）.Compared with 20μg / m L concentration group,the contents of IL-1 and IL-6 in Synovia were much lower in 35μg / m L and 50μg / m L concentration groups. Compared with 50μg / m L concentration group,the reduction was more obvious in35μg / m L concentration group. Conclusion Intra-articular injection with different concentrations of ozone are all effective for treating KOA,however,35μg / m L concentration demonstrates more significant effects on clinical improvement and the levels of inflammatory cytokines. Moreover,35μg / m L concentration is safer,and more suitable for clinical application and popularization.
