中药灌肠治疗慢性溃疡性结肠炎的疗效观察及对血清TNF-α和IL-6的影响

摘　　要：目的观察中药灌肠对慢性溃疡性结肠炎(UC)的疗效及血清TNF-α和IL-6的影响,并与口服美沙拉嗪进行对比。方法将50例UC患者按就诊顺序随机分为中药灌肠组和西药组,每组25例。中药灌肠组给予清热利湿方灌肠,水煎成300m L,取50m L,隔日灌肠1次,连续治疗4周。西药组给予美沙拉嗪口服,1.0g,每天4次,连续服用4周,观察两组患者的临床疗效并用ELISA法检测血清TNF-α和IL-6含量。结果中药灌肠组有效率92%高于对照组有效率72%(P<0.05);两组患者血清TNF-α和IL-6含量均较治疗前下降,且中药灌肠组TNF-α和IL-6含量较西药组下降更明显。结论中药灌肠较口服美沙拉嗪对于慢性UC患者的症状有更好的改善作用,且抑制血清中炎症因子水平的效果更佳,值得在临床推广。Objective To investigate the effects of chronic ulcerative colitis（ UC） by Chinese herb enema and mesalazine its effects on serum TNF-α,IL-6 levels. Methods 50 cases of patients with UC were randomly divided into TCM enema group and western medicine group,25 cases in each group. TCM group received the Chinese herb enema treatment with Qing＇re Lishi decoction,water decocted into 300 m L,50 m L per time for enema,once per day,continuously treated for 4 weeks. Western medicine group took Mesalazine 1. 0g orally,4 times per day,continuously treated for4 weeks. Serum TNF-α,IL-6 levels detected by ELISA assay before and after treatment in both groups. Results The effective rate of TCM enema group were 92%,higher than 72% of drug group（ P〈0. 05）. Serum TNF-α,IL-6 levels were decreased significantly after treatment in both groups,while the levels of TNF-α and IL-6 were lower in TCM enema group than those of western medicine group after treatment. Conclusion Chinese herb enema is more efficient in improving the symptom and serum inflammatory factor of UC patients than using Mesalazine only,it is worthy of widely application in clinical.
