益气摄血汤与孕激素治疗青春期功能失调性子宫出血临床对比研究

摘　　要：目的:对比益气摄血汤加减与孕激素治疗青春期功能失调性子宫出血的临床疗效。方法:选择病例为2012年12月—2014年12月本溪市中医院妇科门诊收治的青春期功能失调性子宫出血患者60例。用随机数字法分为观察组和对照组。观察组:对中医辨证属脾肾两虚兼血瘀型青春期崩漏患者30例,口服本溪市中医院妇科所研制院内制剂益气摄血汤观察其临床疗效。对照组:诊断为青春期功能失调性子宫出血的患者30例予孕激素治疗。采用卡方检验、秩和检验、配对t检验及Fish精确概率检验。比较两种治疗方法的有效率,用药3个月后复发情况及不良反应发生率。结果:观察组:有效率为93.3%。对照组:有效率为86.67%。两组比较P=0.055,无显著性差异,说明两组疗效相同。两组患者停药3个月后复发率比较:P=0.015,有统计学意义。不良反应:观察组无明显不良反应;对照组4例出现恶心、呕吐、头晕等不良反应,2例患者出现血红蛋白进一步丢失,两组不良反应比较:P<0.05,有统计学意义,说明中药治疗青春期功血不良反应少。结论:益气摄血汤加减治疗青春期功能失调性子宫出血的临床疗效好、副作用小、复发率低,值得临床推广应用。Objective： To compare the clinical efficacy of Yiqi Shexue Decoction and progestogen therapy of puberty dysfunctional uterine bleeding. Methods：To select patients in December 2012 to December 2014 in Benxi hospital outpatient service of department of gynaecology treated 60 patients with puberty dysfunctional uterine bleeding. By using the random number method divided patients into observation group and control group. Observation group ： For syndrome differentiation of traditional Chinese medicine belongs to the spleen and kidney deficiency and blood stasis sype, which 30 patients of puberty dysfunctional uterine bleeding. Drinking Benxi hospital of department of gynaecology preparations of Yiqi Shexue Decoction and observe the clinical efficacy. The control group： The Western medicine diagnosis of 30 patients with puberty dysfunctional uterine bleeding of progesterone therapy. By chi-square test, rank and inspection, paired t test and Fish exact probability test. Comparing effective rate, drug relapse after 3 months and the incidence of adverse reactions. Results ： Observation group： the total effective rate was 93.3%. The control group： the total effective rate was 86.67%. Comparing the two groups, P=0.055, there was no significant difference, two groups of curative effect was the same. Two groups of patients stopped after 3 months, recurrence rate comparison ： P=0.015, with statistical significance. Adverse reaction ： Observation group had no obvious adverse reactions. Control group had 4 cases of nausea, vomiting, dizziness and other adverse reactions. 2 cases of patients with hemoglobin further loss. Two groups of adverse reaction ： P〈0.05, was statistically significant. Chinese medicine treatment of adolescent less adverse reaction function. Conclusion ： Yiqi Shexue Decoction treatment of puberty dysfunctional uterine bleeding have good clinical curative effect, less side effects, worthy of clinical popularization and application.
