摘 要:目的:观察直接灸和隔姜灸大鼠"肝俞"穴对原发性肝癌模型大鼠癌前病变期肝功能及肝脏组织形态的影响。方法:将55只雄性Wistar大鼠随机分为正常组(10只)、模型组(15只)、直接灸组(15只)、隔姜灸组(15只)。采用腹腔注射二乙基亚硝胺(DEN)法制备HCC模型;直接灸组、隔姜灸组在造模的同时分别用小艾炷灸、小艾炷隔姜灸"肝俞"穴,3壮/穴,隔日1次,为期10周。在造模结束后,用Elisa法对各组大鼠血清中谷氨酰转肽酶(GGT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、甲胎蛋白(AFP)进行检测,用HE染色观察大鼠肝脏组织形态学变化。结果:造模结束后,与模型组相比,直接灸组GGT、AFP含量显著降低(P<0.05),隔姜灸组GGT、AST、ALP、AFP含量均显著降低(均P<0.05)。肝脏组织切片HE染色光镜下观察,模型组出现肿瘤灶,小叶结构完全破坏,异型性增加;直接灸组,癌细胞数量较少,细胞核异型性低,有少量结节形成;隔姜灸组,细胞核深染、未发现结节。结论:直接灸和隔姜灸都能够降低HCC模型大鼠血清中GGT、AST、ALP、AFP的含量,改善肝功能,减轻肝细胞的损伤,且隔姜灸的效果要优于直接灸组。Objective: To observe the effect of Direct moxibustion and Ginger moxibustion on liver function and liver tissue morphology of primary liver cancer in rats with precancerous lesion. Methods: A total of 55 male Wistar rats were randomly divided into a normal group (10 rats) , a model group (15 rats) ,a Direct moxibustion group ( 15 rats) and a Ginger moxibustion group ( 15 rats). HCC models were prepared by intraperitoneal injection of diethylnitrosamine(DEN). Direct moxibustion group and Ginger moxibustion group were treated by moxa cone at Ganshu point, 3 cones/acupoint, once a day, for a period of 10 weeks. At the end of modeling, glutamyltranspeptidase, aspartatetransaminase and alkaline phosphatase were checked by Elisa. HE staining was used to observe the morphological changes of liver tissues in rats. Results:After modeling, compared with those of the model group,the contents of serum GGT and AFP of the Direct moxibustion group were significantly reduced ( P 〈 0.05 ) , and the contents of serum GGT, AST, ALP and AFP of the Ginger moxibustion group were significantly lower ( P 〈 0.05 ). Under the light microscopic observation of liver tissue stained with HE, the model group had tumor formation; lobular structure was completely destroyed; atypia of hyperchromatic nuclei was increased; Direct moxibustion group had a small amount of liver plate, and atypia of liver cell nuclear was low;Ginger moxibustion group had hyperchromatic nuclei deeply stained, without finding nodules. Conclusion: Direct moxibustion and Ginger moxibustion can reduce the contents of GGT, AST and ALP in serum of HCC model rats and can improve the liver function. The effect of Ginger moxibustion is superior to that of Direct moxibustion.
