单孔胸腔镜手术在老年患者肺切除术中的应用

摘　　要：目的评价单孔胸腔镜手术在老年患者肺切除术中的应用价值。方法分析2015年1月至12月中国医科大学附属盛京医院胸外科行单孔及三孔胸腔镜肺切除术老年患者的临床资料,比较围手术期各临床指标。结果单孔组在切口长度、带管时间、术后第1、3、7天的VAS疼痛评分及术后1个月满意度方面明显优于三孔组(均P<0.05),而在手术时间、术中出血量、淋巴结清扫数目、术后并发症方面无统计学差异(均P>0.05)。结论单孔胸腔镜肺切除术是一种损伤更小、恢复更快且安全可行的手术方式。Objective To evaluate the clinical significance of single-incision video-assisted thoracoscopic surgery（SI-VATS）in elderly patients undergoing pneumonectomy. Methods The clinical data of elderly patients received pulmonary resection by SI-VATS and three-port video-assisted thoracoscopic surgery（TP-VATS）last year were analyzed. Results The SI-VATS group was significantly better than the TP-VATS group in incision length,drainage time,as well as the 1st,3rd,7th day VAS score and one month satisfaction after operation（all P 0.05）. However,no significant difference was found in operation duration,blood loss,lymph node number,or complications（all P 0.05）. Conclusion SI-VATS pulmonary resection is safe,feasible and with less invasive.
