枳实薤白桂枝汤联合低分子肝素治疗肺栓塞临床研究

摘　　要：目的:观察枳实薤白桂枝汤联合低分子肝素治疗肺栓塞的疗效。方法:将2014年2月—2015年2月本院收治的76例肺栓塞患者随机分为对照组和治疗组各38例。对照组给予低分子肝素5 000 U皮下注射治疗,每12 h 1次,连用7 d;治疗组加用枳实薤白桂枝汤治疗,连续治疗15 d。观察两组患者临床疗效及相关指标变化。结果:治疗组PaO_2、PaCO_2水平较治疗前及对照组升高明显,差异有统计学意义(P<0.05);治疗组PT、APTT、TT较治疗前及对照组均有所升高,组间比较差异有统计学意义(P<0.05);治疗组BNP、TNF-α水平降低程度明显优于对照组,差异有统计学意义(P<0.05);治疗组的有效率为92.1%,高于对照组的有效率71.1%,差异有统计学意义(P<0.05)。结论:枳实薤白桂枝汤联合低分子肝素治疗肺栓塞疗效确切,利于改善动脉血气,抗凝作用突出。Objective： To observe the clinical effects of Immature Bitter Orange, Chinese Chive and Cinnamon Twig Decoction combined with low-molecular heparin on pulmonary embolism. Methods： Totally 76 patients with pulmonary embolism hospitalized between February, 2014 and February, 2015 were randomized into control group and treatment group, with 38 cases in each group. Control group were given subcutaneous injection of low-molecular heparin of 5 000 U once every 12 h and for continuous use of 7 d. Treatment group were added with Bitter Orange, Chinese Chive and Cinnamon Twig Decoction for continuous treatment of 15d. The clinical effects and relevant indicators in the two groups were observed. Results： The levels of PaO2 and PaCO2 in treatment group increased when compared with before treatment and control group ; the difference had statistical significance （ P 〈 0.05 ）. The levels of PA, APTT and TT got increased When compared with before treatment and control group; the difference be- tween the two groups had statistical significance （ P 〈 0.05 ）. The decline of the levels of BNP and TNF-α in treatment group was more obvious than that in control group, with statistical significance （P 〈 0.05 ）. The effective rate was 92. 1% in treatment group and 71.1% in control group （P 〈 0.05 ）. Conclusion ： Immature Bitter Orange, Chinese Chive and Cinnamon Twig Decoc- tion combined with low-molecular heparin has definite efficacy on pulmonary embolism, which helps improve the arterial blood ga- ses and has remarkable anticoagulation action.
