清肺通络法治疗儿童肺炎支原体肺炎干咳

摘　　要：邪郁肺络乃儿童肺炎支原体肺炎的病机关键,贯穿整个病程的急性期和迁延期,而热郁肺络则是MPP干咳的病机关键。针对热郁肺络,肺失宣肃,肺气上逆的干咳病机,治疗中以清肺通络为治疗原则,以清肺化痰,清中有润;疏通肺络,开郁散结;肃降气机,肺胃兼顾为其治法。急性期痰热阻络,肺津已伤者,以清肺化痰、肃降肺气为主,滋阴润肺为辅,并佐以通络散结;迁延期痰热未清,肺津耗伤,以滋阴润肺、肃降肺气为主,清肺化痰为辅,并佐以通络散结。根据患儿在不同时期不同临床证候,辨证施治,或以清化为主,或以清润为主,或以肃降为主,或以通络开郁为主,再随症加减,使痰热得清,肺络得通,宣肃得复,诸症得除。The key pathogenesis of children＇s mycoplasma pneumonia （MP） is stagnant pathogens in the lung collaterals, which is throughout the course of the acute period and persisting period. And heat obstructing the lung collaterals is the key pathogene- sis of MP cough. In terms of heat obstructing the lung collaterals, the tung failing to disperse qi and adverse rising of pulmonary qi, the treatment should regard clearing the lung and unblocking collaterals as the therapeutic principle to removing the lung heat and phlegm and moistening the lung, unblock lung collaterals, remove stagnation. Descending qi activity, caring both the lung and stomach are the treatment methods. For the patients whose lung fluid has been hurt by stagnated phlegm and heat in the tung during acute stage, the treatment should focus on clearing lung heat, removing phlegm and descending pulmonary qi, supplemen- ted by nourishing yin and moistening the lung, plus unblocking collaterals and removing stagnation, For the patients whose lung fluid has been hurt by stagnated phlegm and heat in the lung during persisting stage, the treatment should focus on nourishing yin and moistening the lung, and descending pulmonary qi, supplemented by clearing lung heat and removing phlegm, plus unblock- ing collaterals and remove stagnation. According to children＇s different clinical syndromes at different stages, treatment based on syndrome differentiation should be adopted by mainly focusing on clearing lung heat, and removing phlegm, or cleating lung heat and moistening the lung, or descending pulmonary qi, or unblocking collaterals and removing stagnation. Then modification is used according to exact syndromes to clearing heat and phlegm, unblock lung collaterals, release pulmonary qi, and remove all the symptoms.
