摘 要:目的探讨基质金属蛋白酶-9(MMP-9)与孕妇子痫前期发病的相关性。方法将2013年4月-2014年6月在该院住院分娩的孕妇按照其妊娠20周以后的血压以及蛋白尿情况分为实验组(子痫前期66例)和对照组(正常妊娠34例),实验组分为轻度子痫前期组(33例)和重度子痫前期组(33例),应用免疫组化SP法对两组胎盘组织中MMP-9的表达水平进行检测,并测定其光密度值。同时对每个新生儿体重和胎盘重量进行测量和记录。结果在胎盘组织中,MMP-9的光密度值实验组低于对照组(t=4.375,P<0.001),轻度子痫前期组高于重度子痫前期组(t=5.302,P<0.001)。实验组胎盘重量(t=3.181,P=0.002)和新生儿体重(t=3.034,P=0.003)低于对照组,轻度子痫前期组胎盘重量(t=5.128,P<0.001)和新生儿体重(t=4.956,P<0.001)均高于重度子痫前期组。在胎盘组织中,MMP-9的表达水平与胎盘重量及新生儿体重呈正相关。与新生儿体重<2.5 kg者相比,≥2.5 kg者的MMP-9在胚胎组织中的表达水平较高(t=17.121,P<0.001)。结论 MMP-9在孕妇子痫前期的胎盘组织中表达程度低,孕妇病情恶化使其表达程度逐渐降低,表明MMP-9可能和子痫前期的发病有关。Objective To explore the correlation between matrix metalloproteinase- 9( MMP- 9) and onset of preeclampsia in pregnant women. Methods The pregnant women from the hospital from April 2013 to June 2014 were divided into experimental group( including 66 cases with preeclampsia) and control group( including 34 cases of normal pregnancy) according to blood pressure and proteinuria after the 20 th gestational week. The cases in experimental group were divided into mild preeclampsia group( 33 cases) and severe preeclampsia group( 33 cases). Immunohistochemical SP method was used to detect the expression levels of MMP- 9 in the two groups,the optical density values were measured. Weight and placental weight of each neonate were measured and recorded. Results In placental tissue,MMP- 9 optical density value in experimental group was statistically significantly lower than that in control group( t = 4. 375,P 0. 001); MMP-9 optical density value in mild preeclampsia group was statistically significantly higher than that in severe preeclampsia group( t= 5. 302,P〈0. 001). Placental weight and neonatal weight in experimental group were statistically significantly lower than those in control group( t = 3. 181,P = 0. 002; t = 3. 034,P = 0. 003); placental weight and neonatal weight in mild preeclampsia group were statistically significantly higher than those in severe preeclampsia group( t = 5. 128,P〈0. 001; t = 4. 956,P〈0. 001). In placental tissue,the expression level of MMP-9 was positively correlated with placental weight and neonatal weight. Compared with the neonates with weight 2. 5 kg,the expression level of MMP-9 in embryonic tissues of neonates with weight≥2. 5 kg was higher( t = 17. 121,P〈0. 001). Conclusion The expression level of MMP-9 in placental tissue of pregnant women with preeclampsia is low,which decreases gradually with aggravation of the disease,the result indicates that MMP-9 may be related to onset of preeclampsia.
