刘瑞芬辨治多囊卵巢综合征不孕症经验

摘　　要：刘瑞芬认为多囊卵巢综合征不孕症病机为肝、脾、肾三脏功能失调,主要责之肾虚。卵子为生殖之精,其发育成熟需肾精充足,卵子的排出靠肾阳的鼓动。若肾精亏虚,卵子难以发育成熟。肾阳虚损,鼓动无力,气血运行无力而瘀滞胞宫,卵子排出障碍,导致不孕。刘老师认为多囊卵巢综合征所致的不孕症临床上常见肾虚血瘀型,治以补肾活血为主,辅以健脾祛湿、疏肝理气。方中重用紫石英为临证用药一大特色,并酌加血肉有情之品,竣补其虚。以"补、调、温、通"立法,形成独特的中药人工周期,以建立规律的月经周期,中西合参缩短病程,并注重情志调节和孕后保胎。LIU Ruifen believed that the basic pathogenesis of infertility due to polycystic ovary syndrome was related to malfunction of the kidney,spleen and liver,particularly the kidney deficiency. Ovum was the essence of reproduction,its growth and mature depended on that adequate essence of kidney. The release of ovum depended on the agitation of kidney yang. If the kidney essence was insufficient,ovum would be immature. Feeble circulation of qi and blood owing to insufficient of kidney yang and weak agitation,will lead to blood stasis in the uterus which led to infertility. Professor LIU put forward that the most common clinical type was kidney deficiency and blood stasis,and make a more complete and effective treatment plan that was given priority to tonify the kidney and promote blood circulation,invigorate the spleen and eliminate the dampness,soothe the liver qi. Massive use of amethyst（Zishiying） in prescription is her prescription feature according to clinic symptoms and signs. Staging treatment of ＂nourishing,adjustment,warming,restoring＂ according to the different stages of menstrual cycle formed the special artificial cycle of traditional Chinese medicine. Appropriate nutritious medicines were used to nourish body. Combination of Chinese and Western medicine would shorten the disease course. Attention should be paid to emotional regulation and pregnancy after miscarriage.
