摘 要:目的:探讨慢性心力衰竭(CHF)男性患者血浆脂联素(APN)水平的变化及其与脑钠肽(BNP)、肿瘤坏死因子-α(TNF-α)的关系。方法:将60例男性CHF患者设为CHF组,按照纽约心脏病学会(NYHA)分级,患者中Ⅱ级12例、Ⅲ级26例、Ⅳ级22例;60例男性健康查体者作为对照组。应用ELISA法测定两组研究对象的血浆APN、BNP水平;用放射免疫法测定两组研究对象的TNF-α水平,同时进行APN与BNP、TNF-α相关性分析。结果:CHF组患者的APN、BNP、TNF-α明显高于对照组(P均<0.05)。不同病因所致CHF组不同级别患者中的APN水平无统计学差异(P>0.05)。NYHAⅣ患者的血浆APN、BNP、TNF-α明显高于NYHAⅢ及NYHAⅡ患者(均P<0.05)。相关分析显示APN分别与BNP、TNF-α呈正相关(r=0.519,P=0.000;r=0.252,P=0.013)。结论:CHF患者血浆APN水平明显升高,血浆APN水平随其心功能恶化而增高;不同病因所致CHF患者APN水平无统计学差异;APN与BNP、TNF-α、LVMI具有相关性。Objective: To investigate relationships of plasma adiponectin( APN) changes and brain natriuretic peptide( BNP)and tumor necrosis factor-α( TNF-α) of male patients with chronic heart failure( CHF). Methods: 60 male patients with CHF were set as CHF group and classified into NYHA II of 12 cases,NYHA Ⅲ of 26 cases and NYHA IV of 22 cases according to New York heart association functional classification( NYHA),while 60 healthy men were used as control group. The levels of plasma APN and BNP were measured by ELISA and the TNF-α level was measured by radioimmunoassay. Further,the correlations among APN and BNP,TNF-α were analyzed. Results: The levels of APN,BNP and TNF- α of CHF group were significantly higher than those of control group( P〈0.05). There were no statistical differences in the APN level among the CHF patients with different NYHA classes induced by different causes( P〉0. 05). The levels ofplasma APN,BNP and TNF- α of the NYHA Ⅳ patients were significantly higher than those of the NYHA Ⅲ and NYHAⅡ patients( all P〈0.05). The correlation analysis showed APN was positively correlated with BNP and TNF-α,respectively( r = 0.519,P = 0.000; r = 0.252,P = 0.013). Conclusions: In the patients with CHF,the plasma APN levelis significantly elevated,and increases with the deterioration of cardiac function. There are no statistical differences in the APN level among the CHF patients induced by different causes. Moreover,APN is correlated with BNP,TNF-α and LVMI.
