腹腔镜上消化道穿孔修补术的疗效探讨

摘　　要：目的研讨上消化道穿孔患者运用腹腔镜上消化道穿孔修补术治疗的临床适用性及安全性。方法从江苏省苏州市吴江区中西医结合医院2013年1月—2016年1月方便选取58例上消化道穿孔患者进行试验,随机分成Ⅰ、Ⅱ组(各29例),分别接受传统开腹术、腹腔镜上消化道穿孔修补术治疗,评估该两种手术方案的处理效果。结果 1与Ⅰ组相比,Ⅱ组手术患者在操作时间(71.8±10.9)min、术后排气时间(20.8±5.5)h及住院时间(5.2±1.1)d上均显著缩短,在术中出血量(57.0±14.5)m L、术后引流量(133.8±20.6)m L上均显著减少,差异有统计学意义(P<0.05)。2两组在术后均有并发症产生,但Ⅱ组发生率3.4%,显著低于Ⅰ组的24.1%,差异有统计学意义(P<0.05)。结论对上消化道穿孔患者行腹腔镜上消化道穿孔修补术治疗,手术微创、操作时间短,且具有恢复快、安全性好的特点,值得推荐。Objective To study the clinical applicability and safety of patients with digestive tract perforation treated with Laparoscopic upper gastrointestinal perforation repair operation. Methods Convenient selection from January 2013 to January 2016, 58 patients with upper gastrointestinal perforation were selected in Jiangsu Traditional Chinese Medicine Hospital, and randomly divided into Ⅰ,Ⅱ group（29 cases in each group）, they were treated with the traditional open surgery and Laparoscopic upper gastrointestinal perforation repair operation, and then the treatment effect of two kinds of operation scheme were evaluated. Results 1Compared with Ⅰ group, the operation time（71.8±10.9）min, postoperative exhaust time（20.8±5.5）h and hospital stay（5.2±1.1）d of patients in surgery Ⅱ group were significantly shortened, and the intraoperative blood loss（57.0±14.5）m L, postoperative led traffic volume（133.8±20.6）m L were significantly reduced, the difference was statistically significant（P〈0.05）. 2There were postoperative complications in the two groups, but the incidence of Ⅱ group was 3.4%, which was significantly less than 24.1% of the Ⅰ group, the difference was statistically significant（P〈0.05）.Conclusion The patients with digestive tract perforation are treated with Laparoscopic upper gastrointestinal perforation repair operation, with minimally invasive surgery, short operation time, and it is characterized by rapid recovery, good safety,being worth to recommend.
