膨肺吸痰法在机械通气治疗慢性阻塞性肺疾病患者的疗效分析

摘　　要：目的探讨膨肺吸痰法在机械通气治疗慢性阻塞性肺疾病患者的疗效。方法方便选取2016年1—4月该院呼吸ICU收住院的48例行机械通气治疗的慢性阻塞性肺疾病患者,随机分成两组,研究组(n=24例)采用膨肺吸痰法,对照组(n=24例)采用常规吸痰法。观察两组吸痰前后血气分析(SPO2、Pa O2、Pa CO2)以及肺部感染发生率、肺不张发生率指标变化。结果两组患者吸痰后SPO2、Pa O2指标均较吸痰前升高,而Pa CO2较吸痰前下降(P<0.05),研究组较对照组升高或下降的更为明显(P<0.05),研究组肺部感染发生率和肺不张发生率分别是16.67%和12.50%,对照组肺部感染发生率和肺不张发生率分别是45.83%和37.50%,研究组较对照组肺部感染、肺不张的发生率低(P<0.05)。结论膨肺吸痰能有效改善患者血气分析指标,改善通气和换气,预防低氧血症,降低肺不张和肺部感染的发生率,膨肺吸痰法较普通吸痰法有较好的疗效。Objective To study the curative effect of lung dilated suction in mechanical ventilation treatment for patients with chronic obstructive pulmonary diseases. Methods Convenient selection 48 cases of patients with lung dilated suction for mechanical ventilation treatment in ICU in our hospital from January 2016 to April 2016 were randomly divided into two groups with 24 cases in each, the research group adopted the lung dilated suction, while the control group adopted the routine dilated suction, and the blood gas analysis（SPO2、Pa O2、Pa CO2）, incidence rate of pulmonary infection and pulmonary atelectasis before and after suction were observed. Results Two groups of patients after sputum SPO2, Pa O2 index were lower than before sputum aspiration increased, while Pa CO2 was the sputum decreased（P0.05）, the study group compared with the control group increased or decreased more obviously（P0.05）, the study group the incidence of pulmonary infection and the incidence of atelectasis were 16.67% and 12.50% in the control group the incidence of pulmonary infection and the incidence of atelectasis were 45.83% and 37.5%, the research group than in the control group, pulmonary infection and atelectasis of the low incidence rate（P0.05）. Conclusion The lung dilated suction can effectively improve the blood gas analysis indexes, improve the ventilation, prevent the hyoxemia, reduce the incidence rate of pulmonary infection and pulmonary atelectasis, and the curative effect of lung dilated suction is better than that of the common suction.
