摘　　要：目的评价高碳酸血症呼吸衰竭患者经加温湿化高流量氧疗(HFNC)后的疗效。方法 2016年7月2日该院收治1例因慢性阻塞性肺疾病(COPD)并发重度高碳酸血症呼吸衰竭患者,因患者无法耐受无创通气治疗而拒绝使用,后给予HFNC治疗。结果使用HFNC 2 h后,动脉血气p H从7.236升高到7.361,PCO_2由116.5 mm Hg降至80.0 mm Hg、第2天PCO2为60.4 mm Hg。结论对于无法耐受无创通气(NIV)的重度高碳酸血症呼吸衰竭可考虑应用HFNC治疗。Objective To evaluate the curative effect of high-flow warming and humidification oxygen therapy for severe hypoxymic hypercapnic respiratory failure. Methods 1 case of patients with chronic obstructive pulmonary disease and severe hypoxymic hypercapnic respiratory failure admitted and treated in our hospital on July 2, 2016 were selected and the patients refused to use it due to failing to tolerate the non-invasive ventilation and then the patients were treated with HFNC. Results The arterial blood gas p H increased from 7.236 to 7.361, and PCO_2 decreased from 116.5 mm Hg to 80 mm Hg and the PCO_2 was 60.4 mm Hg. Conclusion For patients with severe hypoxymic hypercapnic respiratory failure failing to tolerate the NIV, we can consider to apply the HFNC treatment.
