冠状动脉粥样硬化性心脏病男性患者维生素D浓度的变化及其临床意义

摘　　要：目的使用前瞻性研究的方法,对比观察冠状动脉粥样硬化性心脏病(冠心病)患者血清25-羟维生素D浓度的变化,并研究其临床意义。方法采用化学发光法检测80例冠心病患者与80例对照组患者外周血血清25-羟维生素D浓度,比较两组间的差异以及25-羟维生素D与冠状动脉粥样硬化炎性指标C反应蛋白和白细胞介素-6的相关性。结果冠心病组血清25-羟维生素D浓度显著低于对照组,差异具有统计学意义[(34.3±8.8)nmol/L vs.(51.3±7.7)nmol/L,P<0.05]。冠心病组患者血清25-羟维生素D浓度与C反应蛋白(r=-0.323,P<0.01)、白细胞介素-6(r=-0.198,P<0.01)呈负相关,与高密度脂蛋白浓度呈正相关(r=0.201,P<0.01)。结论冠心病患者血清维生素D浓度明显低下,并呈现缺乏的倾向,提示维生素D的缺乏和不足与冠状动脉粥样硬化的病理进程具有一定的相关性,为未来冠心病的预防和治疗提供新靶点。Objectives To compare the changes of serum concentration of 25-hydroxyvitamin D in patients with coronary artery disease（CAD） and to clarify its clinical significance by a prospective study. Methods Concentrations of 25-hydroxyvitamin D in peripheral blood in 80 patients with coronary heart disease and 80 controls were measured by chemiluminescence method. The differences in 25-hydroxyvitamin D were compared between the two groups.Relations of serum concentration of 25- hydroxyvitamin D, C- reactive protein and interleukin- 6 were also analyzed and compared. Results Serum concentration of 25-hydroxyvitamin D in CAD group was significantly lower than that in control group [（ 34. 3 ± 8. 8） nmol / L vs.（ 51. 3 ± 7. 7） nmol / L, P〈0. 05 ], and it had negative correlations with C- reactive protein（r=-0.323, P〈0.01） and interleukin-6（r=-0.198, P〈0.01） as well as a positive correlation with highdensity lipoprotein（r =0.201, P〈0.01）. Conclusions Serum concentration of vitamin D in patients with CAD is significantly lower than that of patients without CAD, which suggests that deficiency of vitamin D may be associated with the pathological process of coronary atherosclerosis, thus providing a new target for the prevention and treatment of coronary heart disease.
