负压封闭引流技术治疗心脏手术后切口感染的临床疗效观察

摘　　要：目的探讨负压封闭引流(vacuum sealing drainage,VSD)技术治疗心脏术后切口感染的临床疗效。方法选取2010年1月至2015年12月在广东省人民医院进行心脏术后切口感染治疗的260例患者作为研究对象。其中154例患者采用VSD治疗,设为VSD组;106例患者使用普通负压治疗并设为对照组。分析并比较两组的一般临床资料,观察两组术后持续感染、抗生素使用及住院时间及愈合情况。结果两组一般临床资料比较,差异无统计学意义(P>0.05)。VSD组负压使用时间明显短于对照组(P<0.05)。治疗后,VSD组患者持续感染时间、抗生素使用及住院时间显著短于对照组,差异有统计学意义(P<0.05)。VSD组2周愈合患者比例比对照组高,差异有统计学意义(P<0.05);>3周愈合患者比例明显少于对照组,差异有统计学意义(P<0.05)。结论 VSD技术治疗心脏术后切口感染具有良好的临床效果,治疗时间短,伤口愈合快,可以结合临床具体情况进行推广。Objectives To explore the clinical effect of vacuum sealing drainage（VSD） on wound infection after cardiac surgery. Methods From January 2010 to December 2015, 260 patients who received treatment for wound infection after cardiac surgery in Guangdong General Hospital were included. VSD group with 154 patients used VSD and control group with 106 patients used common vacuum. Clinical data, duration of persistent infection,duration of antibiotic use, duration of hospital stay and ratio of concrescence between the two groups were compared.Results Clinical data between the two groups had no significant difference（P〈0.05）. Duration of using vacuum in VSD group was significantly shorter than that in control group（P〈0.05）. Compared with control group, duration of persistent infection, duration of antibiotic use and duration of hospital stay in VSD group were significantly shorter（ P〈0.05）. There was a significant difference in the ratio of concrescence at 2 weeks between the two groups（P〈0.05）.In 3 weeks, the ratio of concrescence in VSD group was much less than that in control group（P〈0.05）. Conclusions VSD is effective for wound infection after cardiac surgery with a short treatment duration and a fast healing process. VSD can be promoted according to suitable clinical conditions.
