临床护理路径应用于小儿毛细支气管炎疗效的Meta分析

摘　　要：目的系统评价国内临床护理路径应用于小儿毛细支气管炎的疗效。方法计算机检索万方数据库、中国知网数据库、维普期刊中文数据库,查找临床护理路径对比常规护理应用于小儿毛细支气管炎疗效的RCT,检索时限均为从建库至2015年12月。由2位研究者按照纳入与排除标准筛选文献、提取资料和评价纳入研究的方法学质量后,采用Rev Man 5.3软件进行Meta分析。结果共纳入9个RCT,共994例患者,Meta分析结果显示:与常规护理组相比,应用临床护理路径可显著提高小儿毛细支气管炎的患者的有效率[OR=5.06,95%CI(2.49,10.28),P<0.000 01],显著缩短患者住院时间[MD=-1.62,95%CI(-3.12,-0.11),P=0.04],显著降低患者住院费用[MD=-444.76,95%CI(-643.50,-246.01),P<0.000 1],显著提高患儿家属对护理工作的满意度[OR=7.46,95%CI(4.21,13.20),P<0.000 01]及健康知识的掌握度[OR=6.27,95%CI(1.36,28.92),P=0.02]。结论临床护理路径应用于小儿毛细支气管炎具有良好疗效,其效果优于常规护理,具有可行性。Objective To systematically evaluate the domestic application effect of clinical nursing pathway in children bronchiolitis .Methods Databases including WanFang , CNKI and VIP Data were searched for the randomized controlled trials （ RCTs ） about the application effect of clinical nursing pathway versus conventional nursing in children bronchiolitis from the establishment to December 2015.Two reviewers independently screened literature according to the inclusion and exclusion criteria , extracted data, and assessed methodological quality of the included studies .Meta-analysis was then performed using RevMan 5.3 software. Results A total of 9 RCTs involving 994 patients were finally included .Meta-analysis showed that： compared with the conventional nursing group , the application of clinical nursing pathway significantly improved the effective rate of the patients with children bronchiolitis [OR=5.06, 95%CI（2.49, 10.28）, P〈0.000 01], significantly shortened hospital stay[MD=-1.62, 95%CI（-3.12,-0.11）, P=0.04], significantly reduced hospitalization expenses[MD=-444.76, 95%CI（-643.50, -246.01）, P〈0.000 1], significantly improved satisfaction with nursing work and master degree of health knowledge of patient families [OR=7.46, 95%CI（4.21, 13.20）, P〈0.000 01;OR=6.27, 95%CI（1.36, 28.92）, P=0.02, respectively].Conclusions Clinical nursing pathway has a good effect on children bronchiolitis .It is feasible and its effect is better than that of conventional nursing .
