抗病毒治疗并发严重不良反应后更换洛匹那韦/利托那韦片的疗效观察

摘　　要：目的:评估获得性免疫缺陷综合征(AIDS)患者抗病毒治疗后并发严重不良反应,更换洛匹那韦/利托那韦片(LPV/r)的疗效。方法:回顾性分析8例AIDS患者接受三联抗病毒治疗后并发严重不良反应,人类免疫缺陷病毒(HIV)RNA已降至<40拷贝/m L,更换LPV/r单药维持治疗后监测患者症状和体征,CD4^+T淋巴细胞以及HIV RNA。结果:8例患者换方案后随访2~4年,不良反应症状好转,HIV RNA均良好控制,CD4^+T淋巴细胞均稳定上升。结论:AIDS患者接受三联抗病毒治疗HIV载量降至检测下限后给予LPV/r单药维持治疗,抗病毒效果良好,而且因核苷类抗反转录酶药物引起的严重不良反应也明显好转。Objective To evaluate the efficacy of lopinavir/ritonavir（LPV/r） monotherapy in acquired immune deficiency syndrome（AIDS） patients who experienced severe adverse reaction after standard antiretroviral therapy（ART）.Methods A retrospective analysis was made in 8 AIDS patients who achieved virological success but experienced serious adverse reactions after standard ART. Patients were given monotherapy of LPV/r, and symptoms, signs, CD4 count, human immunodeficiency virus（HIV） viral load（HIV VL） were monitored. Results Of the 8 patients followed for 2-4 years,severe adverse reactions were relieved after switching to LPV/r monotherapy,HIV RNA was well controlled and CD4~＋T lymphocyte count was increasing gradually. Conclusions LPV/r monotherapy could lead to profound virological response in HIV/AIDS patients who achieved virological success but experienced serious adverse reactions after standard ART. Severe adverse effects of nucleoside analogs could be relieved after switching to LPV/r monotherapy.
