妊娠期高血压综合征、胎膜早破等围生因素对新生儿结局的影响研究

摘　　要：目的探讨胎膜早破、妊娠期高血压综合征等围生因素对新生儿结局的影响研究。方法选取2015年1月-2016年1月本院接生的新生儿560例,均为单胎新生儿,根据母体分娩前有无妊娠高血压综合征分为妊高征组及无妊高征组,评估妊高征组及无妊高征组的新生儿发生先天性心脏病、先天畸形、早产、出生低体重、新生儿窒息、胎膜早破等情况;根据有无出现胎膜早破分为胎膜早破组、无胎膜早破组,评估胎膜早破组及无胎膜早破组发生新生儿先天性心脏病、先天畸形、早产、出生低体重、羊水异常、新生儿窒息等情况。结果妊高征组新生儿发生先天性心脏病、先天畸形、早产、出生低体重、新生儿窒息、胎膜早破等情况显著高于无妊高征组,差异有统计学意义（P〈0.05）;胎膜早破组发生新生儿先天性心脏病、先天畸形、早产、出生低体重、羊水异常、新生儿窒息等情况显著高于无胎膜早破组,差异有统计学意义（P〈0.05）。结论胎膜早破、妊娠期高血压综合征均可使新生儿结局产生不良影响,需做好围生期保健及预防。Objective To study the clinical effect of pregnancy-induced hypertension（PIH） and premature rupture of membrane（PROM） in the neonatal outcome. Methods 560 singleton neonates born in our hospital from January 2015 to i January 2016 were selected as the study objects. All of them were single. The subjects were divided into PIH group and non-PIH group according to the incidence of PIH. The incidence rate of congenital heart disease, congenital malformation, premature birth, low birth weight, neonatal asphyxia, premature rupture of membrane of the two groups were evaluated. the subjects were divided into PROM group and non-PROM group according to the incidence of PROM. The incidence rate of congenital heart disease, congenital malformation, premature birth, low birth weight, abnormal amniotic fluid, neonatal asphyxia of the two groups were evaluated. Results The incidence rates of congenital heart disease, congenital malformation, premature birth, low birth weight, neonatal asphyxia, premature rupture of membrane for PIH group were significantly higher than those of non-PIH group（P 〈 0.05）. The incidence rates of congenital heart disease, congenital malformation, premature birth, low birth weight, abnormal amniotic fluid, neonatal asphyxia of PROM group were significantly higher than those of non-PROM group（P 〈 0.05）. Conclusion PIH and PROM will bring about the adverse effect in the neonatal outcome. It needs to do well with the perinatal nursing care and preventions.
