摘  要:目的探讨口服益生菌联合早期微量喂养对早产儿肠道菌群及免疫功能的影响。方法选择2014年1月~2016年1月间我院收治的早产儿200例作为研究对象,将其随机分成两组,分别定为观察组和对照组,每组100例,对照组采取在早产儿出生24h内开始非营养性吸吮、早期微量喂养,观察组在对照组基础上给予口服益生菌,测量患儿出生后3、7、14d粪便中肠道乳酸杆菌及双歧杆菌,并对两组患儿治疗前后免疫功能进行统计学分析。结果两组治疗前CD3^+、CD4^+、CD8^+、CD4^+/CD8^+T等T淋巴细胞亚群比较差异无统计学意义(P>0.05);观察组患者治疗后CD4^+、CD4^+/CD8^+T等T淋巴细胞亚群均高于对照组治疗后,且比较差异均有统计学意义(P<0.05);观察组患儿治疗3d粪便中双歧杆菌和乳酸菌测定结果与对照组患儿比较差异无统计学意义(P>0.05);观察组患儿治疗7d、14d粪便中双歧杆菌和乳酸菌测定结果均高于对照组,且比较差异均有统计学意义(P<0.05)。结论对早产儿实施口服益生菌联合早期微量喂养,能够有效促进患儿病情恢复,改善早产儿肠道菌群,提高免疫功能,临床价值显著。Objective To investigate the effects of oral probiotics combined with early micro-feeding on intestinal flora and immune function in preterm infants. Methods 200 cases of preterm infants cured in our hospital from January 2014 to January 2016 were selected and randomly divided into observation group and control group with 100 cases in each group. Patients in control group were treated with birth within 24 hours of preterm infants began to nonnutritive sucking, early micro-feeding, and patients in observation group were treated with oral probiotics on the basis of control group. Lactobacilli and bifidobacteria in the stool 3 days, 7 days, 14 days after birth were measured, and the immune function of the two groups of children before and after treatment were analyzed statistically. Results There were no significant differences in lymphocyte subsets(CD3+, CD4+, CD8+, CD4+/CD8+ T) between the two groups before treatment(P 〉 0.05). After treatment, lymphocyte subsets(CD3+, CD4+, CD8+, CD4+/CD8+ T) of observation group were higher than those of control gorup, and the difference has statistical significance(P 〈 0.05). There was no significant difference between the observation group and the control group on determination of bifidobacteria and lactic acid bacteria in 3D feces(P 〉 0.05). The results of Bifidobacterium and Lactobacillus in the feces of the observation group were higher than those of the control group on the 7th and 14 th day after treatment(P 〈 0.05), and the difference was statistically significant(P 〈 0.05). Conclusion The combination of oral probiotics and micro- feeding in preterm infants can improve the recovery of intestinal microflora, and improve the immune function of preterm infants. The clinical value is significant.
