产后出血患者中联合应用宫腔水囊填塞、宫颈钳夹及阴道纱条填塞3种压迫止血方法的价值

摘　　要：目的对比分析联合宫腔水囊填塞、宫颈钳夹及阴道纱条填塞在产后出血治疗中的应用价值。方法随机将2013年1月-2015年12月期间我院收治的120例阴道分娩产后出血患者分为三组,每组40例,其中接受联合宫腔水囊填塞及阴道纱条填塞治疗者为A组,接受联合宫腔水囊填塞及宫颈钳夹治疗者为B组,接受联合宫腔水囊填塞、宫颈钳夹及阴道纱条填塞治疗者为C组。对比三组在产后出血治疗中的应用效果。结果三组间分娩中出血量、产后24h出血量、止血时间及填充物留置时间存在统计学差异（P〈0.05）,C组分娩出血量及产后24h出血量少于A组及B组,止血时间及填充物留置时间短于对照组（P〈0.05）,而A组与B组间比较,差异无统计学意义（P〉0.05）。C组产后24h RBC、Hb水平高于A组及B组（P〈0.05）。同时,三组间产后并发症发生率比较无统计学差异（P〉0.05）。结论联合宫腔水囊填塞、宫颈钳夹及阴道纱条填塞三种治疗方法可显著的提高产后出血的临床治疗效果。Objective To compare and analyze the value of combined use of intrauterine water packing, cervical clamp and vaginal gauze in the treatment of postpartum hemorrhage. Methods 120 patients with vaginal delivery and postpartum hemorrhage cured in our hospital from January 2-13 to December 2015 were randomly divided into three groups（40 cases in each group）. Among them, patients in group A were treated with combined intrauterine water balloon packing and vaginal gauze tamponade, patients in group B were treated with combined intrauterine water balloon packing and cervical clamp, and patients in group C were treated with the combined intrauterine water balloon packing, cervical clamp and vaginal gauze packing. The effect of three groups in the treatment of postpartum hemorrhage were compared. Results There were significant differences in bleeding volume, postpartum hemorrhage volume, hemostasis time and filling time between three groups（P 〈 0.05）. The amount of bleeding and the amount of bleeding in group C were less than those in group A and B（P 〈 0.05）. There was no significant difference between group A and group B（P 〉 0.05）.The levels of RBC and Hb in group C were higher than those in group A and B（P〈 0.05）. The incidence of postpartum complications among the three groups was not statistically significant（P 〉0.05）. Conclusion Combined with intrauterine water capsule filling, cervical clamp and vaginal gauze packing three treatment methods can significantly improve the clinical treatment effect of postpartum hemorrhage.
