EICU老年病人的预后与早期胃肠营养支持的相关性研究

摘　　要：目的分析老年急诊重症监护病房(EICU)病人的预后与早期胃肠营养支持的相关性。方法选取2012年6月至2015年6月期间我院EICU收治的80例病人作为研究对象,将其随机分为观察组和对照组,每组40例。观察组病人于入院第2天接受早期胃肠营养支持,对照组病人接受静脉营养支持。比较2组病人治疗前后的营养指标变化情况,以及2组并发症发生率、死亡率、原发疾病好转情况,呼吸机撤离时间及住EICU时间,分析病人预后与早期胃肠营养支持的相关性。结果 2组病人治疗后的血红蛋白、血清前白蛋白、蛋白摄入量、呼吸机撤离时间及住EICU时间差异均有统计学意义(P<0.05)。观察组死亡率为5.00%,原发病好转率为77.50%;对照组死亡率为20.00%,原发病好转率为55.00%,2组比较,差异均有统计学意义(P<0.05)。病人的血红蛋白、血清白蛋白水平及蛋白摄入量与病人预后相关(P<0.05),结论早期胃肠营养支持可以有效改善老年EICU病人预后。Objective To analyze the correlation of early enternal nutrition supporting with prognosis of gerontal patients in EICU. Methods From June 2012 to June 2015,80 patients in EICU were chosen and randomly divided into study group and control group,with 40 cases in each group.Patients in study group received enternal nutrition supporting,while patients in control group received intravenous nutrition supporting from the 2nd day after admission. The nutrition indexes,the complication incidence,mortality,cure rate of diseases,weaning time of ventilator and EICU duration were compared between two groups. The correlation of early enternal nutrition supporting with prognosis of gerontal patients was analyzed. Results After treatment, the levels of serum hemoglobin and serum prealbumin,protein intake,weaning time of ventilator and EICU duration had significant difference between two groups（ P〈0. 05）. In study group,the mortality was 5. 00%,and the cure rate of disease was 77. 50%; In control group,the mortality was 20. 00%,and the cure rate of disease was 55. 00%.The difference in two groups was of statistical significance（ P〈0. 05）. The levels of hemoglobin,serum prealbumin and protein intake were significantly correlated with prognosis of patients（ P〈0. 05）.Conclusions Early enternal nutrition supporting can improve the prognosis of elderly patients in EICU.
