中医辨证施护改善糖尿病周围神经病变的研究

摘　　要：目的探讨中医辨证施护对糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)病人的护理效果。方法将纳入的80例DPN病人随机分为观察组和对照组,对照组给予常规护理,观察组根据中医体质分型,在辨证施膳基础上给予中医特色护理技术(中药熏洗+穴位按摩+穴位贴敷+艾灸),比较2组病人治疗前后中医症状积分、Toronto临床评分、运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)的改善情况。结果观察组治疗后的中医症状积分(主症积分、次症积分、舌苔脉象积分)较治疗前明显降低,差异有统计学意义(P<0.05);治疗后观察组中医症状积分较对照组均有明显降低,差异有统计学意义(P均<0.05);治疗后观察组的Toronto临床评分(症状分、反射分、感觉试验分)较治疗前明显降低,差异有统计学意义(P<0.05),且治疗后Toronto临床评分较对照组均有明显降低,差异有统计学意义(P均<0.05);观察组治疗后的正中神经和腓总神经的MNCV及SNCV较治疗前明显改善,差异有统计学意义(P<0.05),且治疗后较对照组有明显改善,差异有统计学意义(P<0.05)。结论应用中医辨证施护,可有效改善DPN病人的临床症状。Objective To explore the nursing effect of traditional Chinese medicine（ TCM）syndrome differentiation on diabetic peripheral neuropathy. Methods Eighty patients with DPN were randomly divided into observation group and control group. The control group received routine nursing care,and the observation group received differential TCM nursing technology（ Chinese herbal fumigation＋massage＋ acupointmoxibustion） according to TCM constitution. TCM symptom scores,Toronto clinical scores,motor nerve conduction velocity（ MNCV） and sensory nerve conduction velocity（ SNCV） of two groups were compared before and after treatment. Results The scores of TCM symptom integral and Toronto clinical score（ symptom score,reflex point,sensory test） in observation group after nursing were significantly lower than those before treatment（ P〈0. 05）,and were significantly lower than those of the control group（ P〈0. 05）. The levels of MNCV and SNCV of the median nerve and common nerve in the observation group were significantly improved after treatment（ P〈0. 05）,and which were significantly higher than those in control group（ P〈0. 05）. Conclusions The application of TCM syndrome differentiation and nursing can effectively improve the clinical symptoms of patients with DPN.
