摘  要:目的:探讨血清同型半胱氨酸(HCY)水平与青年急性脑梗死患者组织型纤溶酶原激活物(rt-PA)静脉溶栓疗效的关系。方法:回顾性分析2014年6月—2015年6月收治的青年急性脑梗死应用rt-PA静脉溶栓治疗的患者382例,其中血清HCY水平正常者156例,血清HCY升高者226例,分别记录两组患者治疗前、治疗后24 h及治疗后7 d的美国国立卒中卫生院卒中评分量表(NHISS)评分;治疗前、治疗后30 d、治疗后90 d的改良Rankin量表(mRS)评分。分析血清HCY水平与青年急性脑梗死应用rt-PA静脉溶栓治疗效果的相关性。结果:高血清HCY组患者治疗后的NIHSS评分及mRS评分均高于血清HCY正常组。结论:血清同型半胱氨酸升高是影响青年急性脑梗死患者rt-PA静脉溶栓预后不良的因素之一。Objective:To investigate the relationship between serum homocysteine(HCY) level and the efficacy of recombinant human tissue-type plasiminogen,actilyse(rt-PA) intravenous thrombolysis in young patients with acute cerebral infarction.Methods:Retrospective analysis was performed in 382 patients with young acute cerebral infarction treated with rt-PA intravenous thrombolysis in the department of internal medicine in tianjin huanhu hospital from June 2014 to June 2015.The serum HCY level of normal patients in 156 cases,226 cases of patients with elevated serum HCY.The two groups of patients were recorded before treatment,after treatment 24 h and 7 d national institute of health stroke scale(NHISS) score,before treatment,after treatment 30 d and 90 d modified rankin scale(mRS) score.Analysis of the relationship between serum HCY level and prognosis of patients with acute cerebral infarction treated with rt-PA intravenous thrombolysis.Results:The NIHSS score and mRS score after treatment in the patients with elevated serum HCY were higher than those in the normal group of serum HCY.Conclusion:The increase of serum homocysteine is one of the factors that affect the prognosis of patients with acute cerebral infarction in young patients with acute cerebral infarction rt-PA.
