固本通络方对IgA肾病患者血B细胞活化因子的影响

摘　　要：目的:观察固本通络方对IgA肾病患者血B细胞活化因子(B-cell activating factor,BAFF)的影响。方法:将85例IgA肾病(CKD1~4期)患者随机分为两组,治疗组43例给予固本通络方治疗,对照组42例给予科素亚治疗,疗程4个月。结果:(1)对照组24 h尿蛋白定量降低(P<0.05),治疗组尿红细胞数和24 h尿蛋白定量显著降低(P<0.01),组间疗效比较差异有统计学意义(P<0.05)。(2)治疗组总有效率为74.4%,对照组总有效率为42.9%,组间疗效比较差异有统计学意义(P<0.05)。(3)治疗组血BAFF水平有显著降低(P<0.01),对照组血BAFF水平有降低趋势,但差异无统计学意义(P>0.05);治疗组血BAFF水平的改善显著优于对照组(P<0.01)。(4)治疗组血BAFF水平变化与尿红细胞数(r=0.834 2,P<0.01)、24 h尿蛋白定量(r=0.568 2,P<0.05)呈正相关。结论:固本通络方有效减少IgA肾病患者的血尿和蛋白尿,其机制可能与降低血BAFF水平相关。Objective ： The aim of this study was to investigate the effect of ＂Guben Tongluo formula＂ on the level of serum B cell activating factor （BAFF） in IgA nephropathy patients. Methods：In this randomized controlled clinical study, 85 patients were enrolled. They were randomized into treatment group （43 cases, Guben Tongluo formula） and control group （42 cases, losartan）, with a four - month course of therapies. Results ： Only 24h urinary protein excretion were reduced in control group （ P 〈 0.05 ）, whereas Red blood cell count of urine and 24 h urinary protein excretion were obviously reduced in treatment group （ P 〈 0.01 ）, with significant differences between the two groups （P 〈 0.05）. The total effective rate was 74.4% in treatment group and 42.9% in control group, with a significant difference between the two rates （ P 〈 0.05 ）. In the treatment group, there was a significant decline in the levels of serum BAFF （P 〈 0.01 ）. Conversely, losartan did not simultaneously change the levels of serum BAFF （P 〉 0.05 ）. When compared the results of control group, the levels of serum BAFF were significantly reduced in the treatment group （P 〈 0.01 ）. Besides, the positive relationship between the variety of serum BAFF level and red blood cell count of urine （r = 0. 834 2, P 〈 0.01 ） as well as the level of 24 h urinary protein excretion （ r = 0. 568 2, P 〈 0.05 ） was found. Conclusion： ＂Guben Tongluo formula＂ is effective to improve hematuria and proteinuria in IgA nephropathy patients, due to reduce the level of serum BAFF.
